Zusammenfassung
An je 20 Stämmen vonProteus mirabilis, Escherichia coli undKlebsiella sp. wurde ein ausgeprägter Synergismus von Cefamandol und Tobramycin festgestellt. Durch Zugabe einer subinhibitorischen Konzentration von Tobramycin ließen sich die Cefamandol-MHK- und -MBK-Werte bei allen geprüften Stämmen vonP. mirabilis undE. coli herabsetzen. Im Durchschnitt betrug die Verbesserung beiP. mirabilis für die MHK (Einsaatmenge 104,5 Keime/ml) 2,95 Stufen und für die MBK 4,05 Stufen. Bei höherer Einsaat (106,5/ml) stiegen zwar die Hemmwerte an, der Synergismus betrug wiederum bis zu 5 Stufen für die MHK (Ø 2,05) und bis zu 6 Stufen für die MBK (Ø 4,45). BeiE. coli (nur hohe Einsaat geprüft) wurde die MHK im Durchschnitt um 3,4 und die MBK um 4,3 Stufen der geometrischen Verdünnungsreihe vermindert, im einzelnen zwischen 1 und 7 Stufen. Ein Teil derKlebsiella-Stämme verhielt sich, wahrscheinlich durch die hohe Einsaatmenge bedingt, indifferent. Für die MHK ergab sich eine Reduktion im Mittel um 1,3 Stufen und für die MBK von 3,55 Stufen. Insgesamt überstieg das Ausmaß des Synergismus von Cefamandol und Tobramycin alle bisher mit der gleichen Technik bei anderen Kombinationen gefundenen Werte. Die klinische Bedeutung liegt in der Möglichkeit zur besseren Beherrschung von akut lebensbedrohlichen Infektionen und in der Überwindung von Persistenzphänomenen durch In-vivo-Bakterizidie.
Summary
Studies with 20 strains each ofProteus mirabilis, Escherichia coli andKlebsiella sp. revealed a marked synergism of cefamandole and tobramycin. The addition of a subinhibitory concentration of tobramycin resulted in a reduction in the MIC's and MBC's of cefamandole for all strains ofP. mirabilis andE. coli tested. The MIC forP. mirabilis was improved by 2.95 dilutions and the MBC by 4.05 dilutions on average (inoculum 104.5 bact./ml). When a larger inoculum was used (106.5/ml), the inhibitory values increased, however synergy was again found to lower the MIC by up to 5 dilutions (Ø 2.05) and the MBC by up to 6 dilutions (Ø 4.45). In the case ofE. coli (only a large inoculum was tested), the MIC was reduced by an average of 3.4 and the MBC by 4.3 dilutions of the geometric dilution series, the individual reduction ranging between 1 and 7 steps. Some of theKlebsiella strains showed no noticeable reaction, probably due to the large inoculum used. The mean reduction of the MIC was 1.3 dilutions, that of the MBC 3.55 dilutions. Overall, the synergism of cefamandole and tobramycin exceeded any values determined so far for other combinations by the same technique. The clinical importance lies in the possibility of improving control of dangerous infections and of overcoming persistence phenomena by bactericidal action in vivo.
