Zusammenfassung

Die Behandlung unterteilt sich in ungezielte und gezielte Chemotherapie. Das Konzept einer Chemotherapie bei nachgewiesenem Erreger läßt sich auf der Intensivstation nur selten erfüllen. Bei Mischinfektionen, schwerer Abwehrschwäche oder tiefsitzenden Infektionen ist ein sicherer Erregernachweis unmöglich. Die Chemotherapie orientiert sich in einer derartigen Konstellation am klinischen Status des Patienten, dem möglichen Erregerspektrum sowie der Wirksamkeit und den Nebenwirkungen des angewandten Antibiotikums. Während sich in der Vergangenheit die Chemotherapie auf die Ampicillin/Oxacillin-Kombinationen Cephalotin, Carbenicillin und Gentamicin stützte, werden in Zukunft neue Cephalosporine, Ureidopenicilline und verbesserte Aminoglykoside zur Anwendung kommen.

Treatment is divided into specific and non-specific therapy. The idea of starting chemotherapy when the causative organism has been identified can only rarely be realized in intensive care wards. In mixed infections, severely lowered resistance or deep-seated infections, it is impossible to determine the pathogen positively. In such cases, chemotherapy is chosen in consideration of the patient's clinical condition, the possible pathogen spectrum, and the effectiveness and side-effects of the antibiotic to be used. While the ampicillin/oxacillin combinations, cephalothin, carbenicillin and gentamicin were the agents most frequently used in the past, newer cephalosporins, ureidopenicillins, and improved aminoglycosides will be used in the future.
