Infektionsproblematik bei chirurgischen Intensivpatienten

Zusammenfassung

Den chirurgischen Intensivpatienten gibt es nicht. Die Problematik ist unterschiedlich, je nachdem, ob es sich um Abdominal-, Unfall-, Herz-, Neuro- oder Kinderchirurgie handelt. Ein perfektes Schleusensystem ist z. B. beim herzchirurgischen Intensivpatienten u. U. sinnvoll, da die Gefährdung überwiegend durch Keime des Umgebungsmilieus ausgelöst wird. Beim abdominalchirurgischen Patienten liegt die Infektionsquelle oft mehr beim Patienten und der Art der Operation, so z. B. Infektionen, bedingt durch Insuffizienzen. Wichtig ist eine kritische Einstellung zur Indikation für Intensivtherapie. Der Patient auf dieser Station unterliegt einem höheren Infektrisiko als auf einer Normalstation. Um dieses Infektrisiko möglichst klein zu halten, bedarf es einer laufenden Schulung des Personals im Hinblick auf hygienische Maßnahmen und einer sehr kritischen Einstellung zur Antibiotika-Anwendung auf Intensivstationen.

There is no standard surgical intensive care patient. The problems differ, depending on whether abdominal, trauma, cardiac or pediatric surgery or neurosurgery is concerned. A perfect lock system may be useful, e. g. in the cardiosurgical intensive care patient, because danger comes mainly from environmental bacteria. In abdominal surgery, the source of infection is more often the patient and the kind of operation, e. g. infections due to insufficiencies. A critical attitude regarding the indication for intensive care therapy is important. Patients in this ward are more in danger of infection than are those in normal wards. Two things are required to reduce this risk of infection as far as possible: a continuous training of the staff with regard to hygiene measures, and a very critical attitude relative to the use of antibiotics in intensive care wards.
