Zusammenfassung
Es wird der Temperatureinfluß auf die Gerinnungsparameter untersucht, die bei der quantitativen Blutungsbestimmung (Hämorrhagometrie) erfaßt werden. Bei Temperaturen zwischen 24°C und 37°C wird die Blutungszeit kaum verändert; sie verlängert sich bei Temperaturen unter 20°C; unter 10°C und über 41°C kommt es zu keiner Blutstillung. Bei Kontrollpersonen verdreifacht sich die Blutungsintensität, wenn die Wunde von 37°C auf 43,5°C erwärmt wird. Bei dieser Temperatur ist die Blutungsintensität so hoch wie bei Patienten mit Morbusvon Willebrand-Jürgens bei Raumtemperatur.
Wird eine bereits geschlossene Wunde auf 43,5°C erwärmt, so beginnt sie bei Kontrollpersonen erneut zu bluten. Als Wärmelysiszeit wird das Zeitintervall zwischen Erwärmung der geschlossenen Wunde und Wiederauftreten der Blutung definiert. Die Wärmelysiszeit beträgt bei Kontrollpersonen 1,03 min (Standarddeviation: 0,16 min). Bei Patienten mit thrombozytärer Blutungsneigung ist die Wärmelysiszeit verkürzt; ein Zusammenhang zwischen verlängerter Wärmelysiszeit und Hyperkoagulärnie ist nicht auszuschließen.
Summary
The author investigates the effect of temperature on the coagulation parameters which are determined by the quantitative bleeding test (hemorrhagometry). At temperatures between 24°C and 37°C the bleeding time is hardly changed; it is prolonged at temperatures below 20°C; below 10°C and above 41°C hemostasis does not occur. In control subjects the intensity of bleeding is tripled if the wound is heated from 37°C to 43.5°C. At this temperature, intensity of bleeding is as high as at room temperature in patients withWillebrand-Jürgens disease.
If an already closed wound is heated to 43.5°C it starts bleeding again in control subjects. The interval between heating of the closed wound and reoccurrence of bleeding is defined as the heat lysis time. This heat lysis time is 1.03 min in control subjects (standard deviation: 0.16 min). In patients with thrombocytic hemorrhagic tendencies the heat lysis time is shortened; a connection between prolonged heat lysis time and hypercoagulemia cannot be excluded.
