On the diagnosis of rare paraproteins

Summary

In the 6,500 cases of paraproteinemia we have identified over the last eight years, only approximately 70% of them were detectable by cellulose-acetate and paper electrophoresis techniques. The remaining 30% were detectable only by immuno-electrophoresis, which is also mandatory for the identification of all paraproteins.

Although most of our cases are readily classified, a broad spectrum of analyses is needed for the rare or difficult-to-detect paraproteins. These analyses include tests of serum, diluted and undiluted, and urine, normal and concentrated, with a series of polyvalent and monospecific antisera; and where indicated, materials and methods other than “classic” agar immunoelectrophoresis.

Zusammenfassung

In einem Zeitraum von 8 Jahren wurden 6500 Fälle von Paraproteinämie diagnostiziert. Hiervon wiesen nur etwa 70% einen M-Gradienten in der Trägerelektrophorese auf Zellulose-Acetat-Folie bzw. Papier auf. Die restlichen 30% konnten nur mittels Immunelektrophorese identifiziert werden. Ein breites Spektrum von diagnostischen Ansätzen ist erforderlich, um seltene Paraproteine zu diagnostizieren bzw. zu differenzieren. Diese Techniken schließen Ansätze mit unverdünnten Serumproben sowie nativen und konzentrierten Urinproben gegen eine Reihe polyvalenter und monospezifischer Antiseren ein. In besonderen Fällen kann der immunelektrophoretische Ansatz durch elektrophoretische Trennung in anderen Trägermedien, wie Polyacrylamidgel und Stärke, ergänzt werden. An Hand von typischen Bildern wird eine Reihe seltener Paraproteine dargestellt und besprochen.
