Zusammenfassung
Bei 73 Patienten wurde untersucht, ob sich bei chronischer Anämie verschiedener Ätiologie die Sauerstoffaffinität des Hämoglobins unterschiedlich verhält und durch welche Faktoren das Ausmaß dieser Affinitätsänderung beeinflußt wird. Nach pathogenetischen Gesichtspunkten wurden die Patienten in 6 Gruppen eingeteilt, folgende Parameter wurden bestimmt: Hämoglobinkonzentration, Hämatokrit, P50-Wert (Halbsättigungsdruck), intraerythrozytäre 2,3-Diphosphoglyzeratkonzentations (2,3-DPG) und arterieller pH-Wert. Die Anämie war bei allen Gruppen gleich stark ausgeprägt, geprägt, die arteriellen pH-Werte lagen im Normbereich, nur die Patienten mit renaler Anämie hatten eine metabolische Acidose. Trotz gleich starker Anämie war die kompensatorische Abnahme der Sauerstoffaffinität nicht bei allen Gruppen gleich. Die höchsten P50-Werte wurden bei chronischer Blutungsanämie (P50: 31,1 mm Hg), Anämie bei Leberzirrhose (30,6 mm Hg) und bei hämolytischer Anämie (30,1 mm Hg) gemessen. Die Hämoblastosen und aplastischen Anämien zeigten relativ niedrige P50-Werte (28,2 bzw. 28,6 mm Hg) und hatten damit eine schlechtere funktionelle Anpassung. Bei all diesen Patienten war die Affinitätsabnahme durch einen Anstieg der 2,3-DPG-Konzentration bedingt. In der Gruppe der renalen Anämie jedoch erfolgte die Rechtsverlagerung der Sauerstoffindungskurve (nicht pH-korrigierter P50-Wert: 31,6 mm Hg) durch die Acidose (Bohr-Effekt). Es wird neben der funktionenllen Bedeutung einer Rechtsverschiebung der Sauerstoffbindungskurve auch die Frage diskuriert, ob eine gesteigerte Erythropoese (erhöhte Retikulozytenwerte) und die hypochrome Form der Anämie die kompensatorische Abnahme der Sauerstoffaffinität begünstigen.
Summary
The oxygen affinity of hemoglobin was established in 73 patients with chronic anemia. The question under investigation was whether, in chronic anemias of various pathogenesis, the oxygen affinity of hemoglobin differs and, if so, what factors influence the extent of such alterations in oxygen affinity. The following parameters were determined: Hemoglobin concentration, hematokrit, P50value (half saturation tension), intraerythrocytic 2,3-diphosphoglycerate (DPG) concentration and arterial pH. The patients were divided into 6 groups according to pathogenesis. The degree of anemia was almost equal in all groups. The pH-values were normal, except in the patients with chronic renal failure, in which they were lowered on account of metabolic acidosis. The compensatory decrease of oxygen affinity of hemoglobin differed from group to group, although the mean hemoglobin concentration in all groups was nearly equal. The greatest decrease was observed in iron deficiency anemia (P50: 31.1 mm Hg), in anemia with cirrhosis of the liver (30.6 mm Hg) and in hemolytic anemia (30.1 mm Hg). Hemoblastosis and aplastic anemia showed lower values (28.2, 28.6 mm Hg resp.) which indicate a poor functional adaptation. In all these patients the decrease of oxygen affinity was caused by a rise of the 2,3-DPG concentration.
The oxygen affinity of the patients with chronic renal failure was decreased due to metabolic acidosis (Bohr effect). The “actual” P50value (not corrected to pH 7.4) of this group was 31.6 mm Hg.
The functional significance of displacement of the oxygen dissociation curve to the right is discussed. The question as to whether the compensatory decrease in oxygen affinity is facilitated by an increase in erythropoiesis and hypochromic anemia is also considered.
