Klinische und elektronenmikroskopische Untersuchungen bei lymphoid-plasmazellulärer Retikulose

Zusammenfassung

Bericht über einen Fall von lymphoid-plasmazellulärer Retikulose, dessen Knochenmarkbild beherrscht war von lymphoidzellenähnlichen Elementen. Die elektronenoptische Untersuchung ergab jedoch ausgedehnte Ergastoplasmastrukturen, so daß diese Zellen als Abkömmlinge von Plasmazellen aufzufassen waren. Außerdem waren diese Zellelemente durch einen Mitochondrienreichtum ausgezeichnet, wie er bisher bei keiner ähnlichen Zellform des Knochenmarkes beschrieben wurde. Zytochemisch zeigte sich eine auffällig starke Aktivität der unspezifischen Esterase. Die sedimentationsanalytische Untersuchung ergab eine normale Verteilung der Plasmaproteine. In der Immuno-Elektrophorese war neben einer Verminderung des γ_G- und γ_1A-Globulins vor allem eine Zunahme des α₂-Makroglobulins auffällig. Autoptisch war die atypische lymphoid-plasmazelluläre Infiltration in fast allen Organen nachweisbar.

Summary

Report of a case of lymphoid plasmacellular reticulosis. The bone marrow showed predominantly lymphoid cells. The electron microscopic investigation, however, revealed enlarged ergastoplasmic membranes. Therefore, these cells were interpreted as descendants of plasma cells. Furthermore these cells showed abundant mitochondrias, not describes hitherto in the bone marrow. Cytochemically, there was a strong activity of nonspecific esterase. Ultracentrifugation studies yielded a normal distribution of plasma proteins. In immuno electrophoresis there was a diminution of γ_G and γ_G globuline and an increase of α₂ macroglobulin. The histological examination of autopsy proved the atypical lymphoid plasma cellular infiltration in nearly all organs.
