Zusammenfassung
Bei fünf Kranken mit dekompensierter perniziöser Anämie wurden durch zytogenetische Untersuchung mit Hilfe des direkten Knochenmark-und in einem Fall zusätzlich des Blutkulturverfahrens numerische und strukturelle Anomalien festgestellt und beschrieben.
Eine Hypodiploidie ist ein häufiger Befund und kommt in verschiedenen chromosomengruppen vor. Diese Hypodiploidie wird ebenso als Folge der der perniziösen Anämie zugrunde liegenden DNS-Stoffwechselstörung gedeutet wie eine Reihe von strukturellen Anomalien, nämlich eine Ausbreitung des Centromers, eine Ausdehnung der sekundären Konstriktionen mit Auftreten von Chromatidlücken und-brüchen. Telomerische Assoziationen führten ferner zu di-und multicentrischen Chromosomen.
Endlich fanden sich unvollständige Kontraktionen und Ausbildung von “Riesen-Chromosomen” und häufig eine Vakuolisierung innerhalb des Chromatids.
Eine für die perniziöse Anämie pathognomische Anomalie wurde-in Übereinstimmung mit den Befunden anderer Autoren—nicht gefunden. Die bei perniziöser Anämie von der Norm abweichenden zytogenetischen Befunde reflektieren die der Erkrankung zugrunde liegende DNS-Stoffwechselstörung.
Summary
Direct marrow cytogenetic preparations were examined in five cases of pernicious anemia, exhibiting various morphologica and numerical anomalies. The morphological alterations consisted of increased breakage, vacuolisation of chromatid, enhancement of secondary constriction, incomplete contraction, spreading of centromere and telomere association. The multicentric giant chromosomes were apparantly the result of telomeric fusions. The monosomy which was observed in the hypodiploid cells of the three cases was not confined to any specific group of chromosomes. A specific anomaly which could be chracterised for pernicious anemia has not been found. The structural and the numerical anomalies described seem to be the result of DNA metabolic disorder.
