Über die Wirkungsweise des Anti-D bei der Prophylaxe gegen die Anti-Rh-Sensibilisierung

Zusammenfassung

Von der drei derzeirt diskutierten Theorien zum Wirkungsmechanismus von Anti-D vermag die der klonalen Ablenkung den Beobachtungen in der Praxis und im Experiment bis jetzt am besten gerecht zu werden. Mit ihr. läßt sich u. a. verstehen, warum die Wirkung von Anti-D nach Antigeneinschwemmung zeitlich begrenzt ist,

Warum eigenproduzierte Antikörper und diejenigen des erbgleichen Zwillings keinen immunosuppressiven Effekt haben,

Warum während einer Schwangerschaft—bei entsprechender Konstellation — nur selten die Grundlage für eine Anti-D-Sensibilisierung gelegt wird, und warum bei ABO-Heterospezifität zwischen Mutter und Kind eine Anti-D-Sensibilisierung nur verhältnismäßig selten eintritt.

Summary

Mainly three theories are under discussion how anti-D actis in anti-Rh prophylaxis. Among these the theory of “Clona deflection” shows the best accordance with clinical observations and experimental findings. Through this theory it becomes understandable

Why the efficiency of anti-D after birth or maltransfusion is limited temporally, why selfmade anti-D and the anti-D of the identic twin do not have an immunosuppressive effect,

Why anti-D priming of the mother by the child's red blood cells during Rh incompatible pregnancies (before delivery) is seldom, and why anti-D sensitization of the mother is rare in cases of mother-child's incompatibility in the ABO system.
