Summary
In 21 patients suffering from immunohemolytic anemia with incomplete warm autoantibodies immunoglobulins and β1A-globulin were determined in the serum. 16 patients were controlled over a longer period of time or the entire course of the disease.
At the time of maximum hemolysis dysimmunoglobulinemia was observed in 57%. Immunoglobulin alterations concerned mainly IgG, but also IgM and only in three cases IgA.
There was no statistical correlation between the type of dysimmunoglobulinemia and autoantibody immunoglobulin class or type of the disease. Clinical improvement was accompanied by normalization or a tendency toward normalization of serum immunoglobulins independent of the amount of autoantibodies.
A significant correlation was found between complement fixation to red cells and decrease in serum β1A-globulin.
It is concluded that dysimmunoglobulinemia might be only a concomitant symptom rather than an etiologic factor in immunohemolytic anemias with incomplete warm autoantibodies.
Zusammenfassung
Bei 21 Patienten mit immunhämolytischer Anämie vom Wärmetyp wurden die Immunglobuline und β1A-Globuline im Serum bestimmt. 16 Patienten wurden über längere Zeit oder den gesamten Verlauf der Erkrankung kontrolliert.
Zum Zeitpunkt maximaler Hämolyse wurde in 57% eine Dysimmunglobulinämie beobachtet. Die Veränderungen der Immunglobuline betrafen vor allem IgG, aber auch IgM und nur in 3 Fällen IgA.
Es fand sich keine statistisch gesicherte Korrelation zwischen Art der Dysimmunglobulinämie und Immunglobulinklasse der Antikörper oder Erkrankungsform.
Die klinische Besserung war, unabhängig von der nachweisbaren Autoantikörpermenge, mit einer Normalisierung oder Tendenz zur Normalisierung der Serumimmunglobuline verbunden.
Es fand sich eine signifikante Korrelation zwischen Komplementbindung an die Erythrozyten und Erniedrigung von β1A-Globulin im Serum.
Aus unseren Befunden wurde geschlossen, daß die Dysimmunglobulinämie eher ein Begleitsymptom als einen ätiologischen Faktor der autoimmunhämolytischen Anämie vom Wärmetyp darstellt.
