Summary
The currently recognized staging classification of malignant lymphoma, which was formulated in Ann Arbor in 1971, is a result of the revision and extension of the 1965 Rye Classification.
It can be distinguished from the latter,first by a division of staging determination into two parts, the “Clinical” and the “Pathological”,secondly by the inclusion of findings which make it possible to apply staging classification not only to Hodgkin's disease, as was the case with the Rye Classification, but also to all malignant lymphoma, andthirdly by the incorporation of new findings gained in the meantime. p] ad 1. While the previous clinical staging classification was based on the results of predominantly clinical investigation methods (clinical, radiological, laboratory and primary histological), pathological staging additionally emphasized surgical investigation methods, especially laparotomy combined with splenectomy in the evaluation. p] ad. 2 The fact that the origin of the disease in extranodal organs, as is frequently the case with non-Hodgkin's as opposed to Hodgkin's lymphomas, has nearly the same or even a better prognosis as a primary lymph node finding; it permits to rank equally both forms of primary nodal and primary extranodal involvement. p ad 3. The following points, established after 1965, were included in the staging classification:

(a)
There are two forms of secondary organ involvement: locally limited organ involvement, which is caused by invasion of the disease from neighbouring involved lymph nodes and is having the same prognosis as lymph node involvement of the same extent, and disseminated organ involvement, arising from hematogenous dispersal and with an unfavourable prognosis.

(b)
As the spleen is a preferred starting-point for hematogenous dispersal, it plays a particular role regarding the spread of the disease.

(c)
In contrast to weight loss, itching has no prognostic significance.
Laparotomy combined with splenectomy is not only a staging but also a therapeutic measure of prophylactic efficacy.
Zusammenfassung
Die heutige gültige Stadieneinteilung der malignen Lymphome wurde 1971 in Ann Arbor formuliert. Sie stellt eine Überarbeitung und Erweiterung der Ryeschen Klassifikation von 1965 dar.
Von dieser unterscheidet sie sicherstens durch eine Aufteilung in zwei Formen der Stadienbestimmung, dem „Clinical“ und „Pathological Staging“,zweitens durch Aufnahme von Befunden, die es ermöglichen, die Stadieneinteilung nicht nur, wie die Ryesche Klassifikation, auf die Hodgkinsche Erkrankung, sondern auf alle malignen Lymphome anzuwenden unddrittens die Aufnahme zwischenzeitlich gemachter neuer Erkenntnisse.
ad 1. Während die bisherige klinische Stadieneinteilung auf den Ergebnissen der vorwiegend klinischen Untersuchungsverfahren (Klinik, Radiologie, Labor und primäre Histologie) beruht, wurden beim Pathological Staging weitere vorwiegend chirurgische Untersuchungsverfahren, insbesondere die Laparotomie mit Splenektomie, in die Beurteilung einbezogen.
2. Die Feststellung, daß der Krankheitsbeginn in extranodalen Organen, wie er bei den Non-Hodgkin-entgegen den Hodgkin-Lymphomen häufig vorkommt, die annähernd gleiche oder bessere Prognose wie ein primärer Lymphknotenbefund hat, macht es möglich, beide Formen des primär nodalen und primär extranodalen Befalls gleichwertig einzuordnen.
ad 3. Folgende seit 1965 gemachte Feststellungen wurden aufgenommen:

a)
Es gibt zwei Formen des sekundären Organbefalls, den lokal limitierten Organbefall, der durch Invasion von benachbarten befallenen Lymphknoten entsteht und eine vergleichsweise gute Prognose hat und den disseminierten Organbefall, der durch hämatogene Aussaat entsteht und eine vergleichsweise schlechte Prognose hat.

b)
Die Milz spielt, da sie bevorzugter Ausgangsort hämatogener Streuungen ist, eine besondere Rolle innerhalb der Ausbreitung der Erkrankung.

c)
Der Juckreiz hat keine, die Gewichtsabnahme dagegen prognostische Bedeutung.
Die Laparotomie mit Splenektomie ist nicht nur eine Staging, sondern auch eine therapeutische Maßnahme mit prophylaktischer Wirksamkeit.
