A review of 128 cases of posterior mediastinal goiter

Abstract

In 9,100 thyroid operations performed between 1935 and 1975, the author treated over 1,300 patients with intrathoracic goiters, 128 of whom had posterior mediastinal goiters. In all patients, the posterior mediastinal goiter arose from the thyroid gland in the neck. The main symptoms were those resulting from compression of the trachea, bronchi, esophagus, and great vessels. Chest x-rays showing displacement of mediastinal structures were the most helpful diagnostic studies. It was possible to remove the goiter entirely through a cervical approach in 122 of the 128 patients. Six patients required a combined cervicothoracic approach after exploration through the neck showed that it was not possible to safely perform a thyroidectomy. There was only one death in this group of 128 patients, all of whom were over the age of 50.

Résumé

Sur 9,100 interventions de chirurgie thyroïdienne faites entre 1935 et 1975, 1,300 concernaient des goitres intra-thoraciques, dont 128 pour goitre dans le médiastin postérieur. Dans tous les cas, ce goitre médiastinal postérieur s'était développé au départ de la thyroïde en position normale, cervicale. Les principaux symptomes étaient dus à la compression de la trachée, des bronches, de l'oesophage ou des gros vaisseaux. L'examen le plus utile au diagnostic a été la radiographie de thorax montrant le déplacement des organes du médiastin. Le goitre a pu être enlevé par voie cervicale pure dans 122 cas; dans 6 cas seulement, un abord cervico-thoracique a été nécessaire, l'exploration du cou ayant montré que la résection par voie cervicale ne pouvait être réalisée ou était potentiellement dangereuse. Il n'y a eu qu'un seul décès parmi ces 128 malades, tous âgés de plus de 50 ans.
