Riassunto
In 21 pazienti affetti da epatite virale acuta (a necrosi focali) sono state effettuate prove di carico con glucosio per via orale e i.v. Nel 50 % dei pazienti è stata dimostrata una diminuzione netta della tolleranza al glucosio. I risultati del test della tolbutamide e la determinazione della insulinemia (IRI), in parallelo con quella della glicemia, dopo carico con glucosio inducono a ritenere che questa turba del ricambio glicidico sia dovuta a depressione dell'attività periferica dell'insulina, probabilmente per l'intervento di antagonisti di origine endocrina o metabolica. Tra questi, sembrerebbero interessati gli acidi grassi liberi del sangue, sempre aumentati rispetto alla norma in condizioni basali e infranormali per tutta la durata dell'iperinsulinemia dopo carico orale nei pazienti con diminuita tolleranza al glucosio.
Résumé
Chez 21 sujets atteints d'hépatite virale aiguë (à nécroses focales) ont été pratiquées des surcharges orale et i.v. de glucose. Chez la moitié des patients on a mis en evidence une diminution nette de la tolérance au glucose. Les résultats du test à la tolbutamide, et de la détermination de l'IRI (pratiquée parallèlement à celle de la glycémie après surcharge de glucose) laissent penser que ce trouble du métabolisme glucidique est dû à la dépression de l'activité périphérique de l'insuline, provoquée probablement par des antagonistes d'origine endocrinienne ou métabolique. Parmi ces derniers, semblent être en cause les acides gras libres hématiques, qui sont toujours augmentés en conditions basales et en dessous de la normale pendant toute la durée de l'hyperinsulinémie après surcharge orale de glucose chez les sujets avec réduction de la tolérance au glucose.
Resumen
En 21 pacientes afectos de hepatitis aguda de virus (con necrosis focales) se han efectuado pruebas de carga con glucosa por vía oral y endovenosa. En el 50 % de los pacientes se ha observado una disminución neta de la tolerancia de glucosa. Los resultados del test de la tolbutamida y la determinación de la insulinemia (IRI), en paralelo con la de la glicemia, tras carga de glucosa inducen a suponer que la turbación del metabolismo glucídico se deba a una depresión de la actividad periférica de la insulina, probablemente por la intervención de antagonistas de orígen endócrino o metabólico. Entre estos, parecerían interesados los ácidos grasos libres de la sangre, siempre aumentados respectos a la normalidad en condiciones básicas o infranormales durante toda la duración de la hiperinsulinemia tras carga oral en pacientes con disminución de la tolerancia a la glucosa.
Zusammenfassung
Bei 21 Patienten mit akuter Virushepatitis (mit Fokalnekrosen) wurden orale und intravenöse Belastungsproben mit Glukose durchgeführt. Bei 50 % der Patienten wurde eine eindeutig verminderte Glukosetoleranz nachgewiesen. Die Ergebnisse des Tolbutamidtests und der Bestimmung des Insulinblutspiegels (IRI), zusammen mit der des Blutzuckerspiegels, nach Glukosebelastung geben zu der Vermutung Anlass, dass die Störung des Kohlehydratstoffwechsels durch eine Verminderung der peripheren Aktivität des Insulins verursacht wird, die ihrerseits wahrscheinlich auf Antagonisten hormonellen oder metabolischen Ursprungs zurückzuführen ist. Unter diesen scheinen die freien Fettsäuren des Blutes von besonderem Interesse, welche in der Ausgangslage stets gegenüber der Norm vermehrt sind und bei Patienten mit verminderter Glukosetoleranz während der ganzen Zeit der Hyperinsulinämie nach oraler Belastung unterhalb der Norm liegen.
Summary
In 21 patients with acute viral hepatitis (with focal necroses) oral and intravenous glucose loading tests were carried out. In 50 % of patients a clear-cut decrease in glucose tolerance was found. The results of the tolbutamide test and blood insulin assay (IRI), together with that of glycemia after glucose loading lead one to suggest that this disorder of glucose metabolism is due to a depression of peripheral insulin activity, probably due to the action of antagonists of endocrine or metabolic nature. Among these, the plasma free fatty acids would seem to be involved: they are always increased in comparison to normal values in basal conditions and their level is below normal for the whole duration of hyperinsulinemia after oral loading in patients with impaired glucose tolerance.
