Gli AA. hanno studiato il metabolismo del glucosio in 9 soggetti acromegalici ed in un gruppo di controlli, effettuando la cinetica del glucosio secondo Conard-Bernier. Questa indagine ha permesso di osservare che nell'acromegalico, quando ancora non si è sviluppato un diabete mellito conclamato, esiste un modesto aumento della glicemia (0,88 contro 0,80 g ‰) e dello spazio di distribuzione del glucosio (l 22 contro 15). Ilpool extracellulare è nettamente aumentato (g 19,62 contro 12,08), mentre l'utilizzazione globale del glucosio non è significativamente diminuita (2,52 mg/min/kg contro 2,81). Il livello basale dei NEFA non è particolarmente elevato nell'acromegalia. La caduta dei NEFA si svolge regolarmente dopo il carico endovenoso, mentre la quantità di glucosio utilizzata durante le prove è francamente ridotta. Gli AA., dopo aver discusso il significato delle loro osservazioni, concludono affermando che nell'acromegalia, ancor prima che si sviluppi un diabete manifesto, esiste un'alterazione della utilizzazione glucidica, che però in questa fase è compensata dall'aumento della glicemia e dello spazio di distribuzione del glucosio. L'utilizzazione del glucosio a livello del tessuto adiposo, invece, sembra non essere ostacolata.

Les AA. ont étudié le métabolisme du glucose dans 9 sujets acromégaliques et dans un groupe de contrôle, ils ont effectué la cinétique du glucose après Conard-Bernier. Cette recherche a permis d'observer que chez le sujet acromégalique, avant que le diabète soit manifeste, existe une modeste augmentation de la glycémie (0,88 contre 0,80 ‰) et de la distribution du glucose (l 22 contre 15). Lepool extracellulaire est nettement augmenté (g 19,62 contre 12,08), tandis que l'utilisation globale du glucose n'est pas significativement diminuée (2,52 mg/min/kg contre 2,81). Le niveau basale des NEFA n'est pas particulièrement élevé dans l'acromégalie. La chute des NEFA est régulière après la charge intraveineuse, tandis que la quantité du glucose utilisée pendant l'examen est très réduite. Les AA., après avoir discuté les résultats obtenus, affirment que dans l'acromégalie, aussi avant que le diabète soit manifeste, existe une altération de l'utilisation des glycides, qui est compensée, à ce moment, par l'augmentation de la glycémie et de la distribution du glucose. L'utilisation du glucose au niveau du tissue adipeux, semble au contraire n'être pas obstaculée.

Los AA. han estudiado el metabolismo de la glucosa en 9 sujetos acromegálicos y en un grupo de controles, efectuando la cinética de la glucosa según Conard-Bernier. Esta investigación ha permitido observar que, en el acromegálico, cuando no se ha desarrollado todavía una diábetes mellitus manifiesta existe un aumento modesto de la glucemia (0,88 contra 0,80 g ‰) y del espacio de distribución de la glucosa (l 22 contra 15). Elpool extracellular se halla aumentado en forma neta (g 19,62 contra 12,08) mientras la utilización global de la glucosa no se halla disminuída en forma significativa (2,52 mg/min/kg contra 2,81). El nivel basal de los NEFA no se encuentra muy elevado en la acromegalia. La caída de los NEFA se verifica regularmente después de la carga endovenosa, mientras la cantidad de glucosa utilizada durante la prueba está francamente reducida. Los AA., luego de haber discutido sobre la significación de sus observaciones, concluyen afirmando que en la acromegalia, antes que se desarrolle una diábetes manifiesta, existe una alteración de la utilización glucídica, la cual, empero, en esta fase está compensada por el aumento de la glucemia y del espacio de distribución de la glucosa. La utilización de la glucosa a nivel del tejido adiposo, por el contrario, parece que no está obstaculizada.
Zusammenfassung
Die AA. erforschten den Glukose-Stoffwechsel bei 9 Akromegalikern und in einer Kontrollgruppe nach der Methode von Conard-Bernier. Diese Untersuchung gestattete die Beobachtung, dass beim Akromegaliker, wenn sich noch nicht ein offener Diabetes entwickelt hat, maessige Steigerung des Blutzuckers (0,88 gegen 0,80 g ‰) sowie des Glukose-Verteilungsraumes (l 22 gegen 15) besteht. Der extrazellulaerepool ist deutlich gesteigert (g 19,62 gegen 12,08) waehrend die Globalverwertung der Glukose nicht signifikant vermindert ist (2,52 mg/min/kg gegen 2,81). Der Blutspiegel der NEFA liegt bei der Akromegalie nicht besonders hoch. Der Abfall der NEFA vollzieht sich regelmaessig nach der i.v. Belastung waehrend die Quantitaet der waehrend der Probe verwerteten Glukose deutlich reduziert ist. Nach Diskussion der Bedeutung der gemachten Beobachtungen kommen die AA. zu der Schlussfolgerung, dass bei der Akromegalie, noch vor der Entwicklung eines manifesten Diabetes, eine Veraenderung der Glukose-Verwertung besteht, die jedoch in dieser Phase durch die Zunahme der Glykaemie und des Glukoseverteilungsraumes kompensiert wird. Die Verwertung der Glukose auf der Hoehe des Fettgewebes hingegen scheint nicht beeintraechtigt.
Summary
The glucose metabolism was studied by the Conard-Bernier method in 9 acromegalic patients and in a group of controls. The investigation showed that before the onset of frank diabetes mellitus, acromegalic subjects exhibit a small increase in blood glucose (0.88 against 0.80 g ‰) and in the glucose distribution space (l 22 against 15). There is marked increase in the extracellular pool (g 19.62 against 12.08) and no significant decrease in global glucose utilization (2.52 mg/min/kg against 2.81). The basal NEFA level is not particularly elevated in acromegaly. The fall of NEFA after i.v. load is regular but the quantity of glucose utilized during the test is clearly below normal. The significance of these findings is discussed and it is concluded that in acromegaly glucose utilization is altered before the onset of diabetes but that in this early stage the alteration is offset by the rise in blood glucose and by the increase in the glucose distribution space. Glucose utilization in the adipose tissue does not seem to be affected.
