Differentialdiagnose und operative Therapie mediastinaler Erkrankungen

Zusammenfassung

An Hand von 43 eigenen Fällen werden Symptomatik, Differentialdiagnose, operative Therapie und Ergebnisse bei verschiedenen mediastinalen Erkrankungen geschildert. Bei den Verletzungen des Mediastinums wird besonders auf die Problematik in der Behandlung der verschiedenen Ursachen des mediastinalen Schockzustandes hingewiesen. Die Therapie der akuten, phlegmonösen Mediastinitis ist trotz Operation und antibiotischer Behandlung noch nicht zufriedenstellend. Frühest möglicher Zeitpunkt der Operation erscheint der beste Weg.

Bei der Diagnostik mediastinaler Tumorprozesse ergibt sich eine deutliche Verbesserung der präoperativen Diagnosestellung durch Phlebographie, Isotopenradiogramm und Mediastinoskopie.

Bei deroperativen Behandlung erscheint es wichtig daraufhinzuweisen, daß die Malignisierungsrate der benignen Tumoren so hoch ist, daß jeder Mediastinaltumor operationsbedürftig ist. Besondere Beachtung wird auf die Tatsache gelegt, daß bei jüngeren Patienten auch ungünstig erscheinende maligne Mediastinaltumoren und metastatische Prozesse im Mediastinum erfolgreich angegangen werden können. Die Kombination von Operation, Bestrahlung und cytostatischer Therapie verbessert die Ergebnisse.
