Zusammenfassung
Mittels einer Filtrationstechnik für Vollblut mit Filtern mit einem Porendurchmesser von 5 µm wurde die Erythrozytenverformbarkeit von Blutkonserven über einen Zeitraum von 6 Tagen untersucht. Innerhalb von 12 Stunden nimmt die Filtrabilität von Vollblut aus Blutkonserven um ca. 50% ab, nach 6 Tagen beträgt der Verlust an Filtrabilität ca. 98%. Der Zusatz von Pentoxifyllin zu alternden Blutkonserven (20 µg/ml Blut) verbessert die Filtrabilität wieder, wobei der Anstieg der Erythrozytenverformbarkeit statistisch signifikant wird, wenn die Blutproben, versetzt mit Pentoxifyllin, für 2.5 Stunden bei 37°C inkubiert werden. Die Ursache dieses Effektes wird in einer Zunahme des erythrozytären ATP-Gehaltes vermutet. Die Messung der Erythrozytenverformbarkeit bei 29 klinisch gesunden Probanden unter Berücksichtigung der Risikofaktoren Zigarettenrauchen und orale Kontrazeptiva ergab, daß männliche Raucher eine signifikant niedrigere Erythrozytenverformbarkeit haben (p<0,001) als männliche Nichtraucher und daß ebenso alle männlichen und weiblichen Raucher eine reduzierte Erythrozytenverformbarkeit (p<0,05) im Vergleich zu einem Kollektiv nichtrauchender Männer und Frauen aufweisen. Die Bedeutung der verminderten Erythrozytenverformbarkeit bei Rauchern als Faktor einer gestörten Mikrozirkulation und mögliche Konsequenzen für die Entwicklung einer Gefäßkrankheit werden diskutiert.
Summary
By means of a filtration technique for whole blood with microfilters with pores of 5 µm in diameter the flexibility of red cells from stored blood was investigated over a period of six days. Within 12 hrs the filtrability of blood from stored blood decreases about 50%, after six days the loss in filtrability amounts to approximately 98%. The addition of pentoxifylline to aging blood (20 µg/ml of blood) improves the filtrability. The increase in red cell flexibility becomes statistically significant when the blood samples containing pentoxifylline are incubated for 2, 5 hrs at 37°C. This effect is assumed to be dependent on an intra-erythrocytic ATP-increase. The measurement of red cell flexibility in 29 healthy volunteers with special attention to the risk factors “cigarette smoking” and “oral contraceptives” revealed that in male smokers red cell flexibility is significantly (p<0.001) decreased in comparison to male non-smokers. Also the comparison of all male and female smokers against all non-smokers of both sexes results in a significantly decrease (p<0.05) in red cell flexibility. The importance of reduced red cell flexibility in smokers as a factor possibly contributing to a disturbance of the microcirculation and its consequences for the development of arterial occlusive disease are discussed.
