Zusammenfassung
1. Die mittlere intracelluläre Wasserstoffionenkonzentration von Erythrocyten betrug in 106 Blutproben von Patienten ohne Störungen des Wasser-, Elektrolyt- oder Säuren-Basenhaushaltes 7,27±0,003 und die extra-intracelluläreph-Differenz 0,12±0,003. Diese Werte entsprechen tierexperimentellen Ergebnissen anderer Untersucher an Muskel-und Gehirnzellen von Warmblütern.
2. Die Wasserstoffionen verteilen sich unter physiologischen Umständen auf beiden Seiten der Erythrocytenmembran wie frei diffusible Ionen in einem Donnan-System. Demzufolge ändert sich die intracelluläre Wasserstoffionenkonzentration unter anderem in Abhängigkeit von der Proteindissoziation.
3. Störungen erfahren diese gesetzmäßigen Konzentrationsverhältnisse durch Freisetzung von Säureäquivalenten aus dem Zellstoffwechsel, durch Änderung der Anionenpermeabilität der Zellmembran und durch Ionenabtausch an der Zellmembran. An anderen Körperzellen (z. B. Magenmucosa) kann außerdem ein aktiver Wasserstoffionentransport das intra-extracelluläre Reaktionsgefälle modifizieren.
4. Die praktisch-klinische Bedeutung dieser Überlegungen wird für die Behandlung der metabolischen Acidose und am Beispiel der Ammoniumintoxikation erörtert.
