Summary
(1) The enzyme thymidylate synthetase was investigated in normal and pathological cells, mainly leukemic cells of the blood and of the bone marrow. A significant correlation between enzyme activity and morphological symptoms was only demonstrable in the bone marrow of non leukemic patients and of chronic granulocytic leukemia. Compared with the dihydrofolate reductase and other enzymes of the folate-“C1”-metabolism the specific activities of thymidylate synthetase are several times higher. (2) In mature leukocytes and in lymphocytes of chronic lymphatic leukemia only minimal activities of thymidylate synthetase are detected. The enzyme activity is increased in immature myeloic cells of the blood in acute leukemia, chronic granulocytic leukemia and in osteomyelofibrosclerosis. In leukemic patients the values in the bone marrow cells are higher than in the peripheral cells. There is not much difference in regenerating non-leukemic cells and leukemic cells of the bone marrow. Therefore it can be assumed, that high activities of thymidylate synthetase are characteristic not only for leukemic cells but for all immature—including normal undifferentiated—cells. (3) By determination of thymidylate synthetase judgement of therapeutic effects during treatment of chronic myelocytic leukemia with cytostatic drugs or by radiation of the spleen and of cytotoxic side effects during treatment of malignoma with 5-fluor-uracil is possible. As a compensation of vitamine B12 deficiency in pernicious anemia a decrease of the enzyme activity during the first days of substitution therapy is demonstrated.
Zusammenfassung
1. Die Thymidylat-Synthetase wird in normalen und pathologischen, insbesondere leukämischen weißen Blut- und Knochenmarkzellen untersucht. Eine signifikante Korrelation zwischen biochemischen Veränderungen und morphologischen Befunden besteht lediglich für normale Knochenmarkzellen sowie für Markzellen bei chronischen Myelosen. Die spezifischen Enzymaktivitäten der Thymidylat-Synthetase sind im Vergleich zur Dihydrofolat-Reductase und anderen Enzymen des Folsäure-„C1“-Stoffwechsels um ein Vielfaches niedriger. 2. In ausgereiften Leukocyten und in Lymphocyten bei chronischen Lymphadenosen sind nur minimale Aktivitäten der Thymidylat-Synthetase nachweisbar. Dagegen finden sich in unreifen myeloischen Zellen des Blutes bei akuten und chronischen Leukämien sowie bei Osteomyelofibrosklerose deutlich erhöhte Aktivitäten. Bemerkenswert ist, daß die Enzymaktivitäten in den Markzellen bei Leukämien stets erheblich höher sind als in den peripheren leukämischen Zellen. Im Durchschnitt fast ebenso hohe Aktivitäten werden in normalen bzw. hyperregenerierenden Zellen des Knochenmarkes bestimmt, woraus ersichtlich ist, daß ein erhöhter Substratumsatz nicht nur für Leukämiezellen, sondern für alle unreifen, also auch für normale noch undifferenzierte Zellen kennzeichnend ist. 3. Die Bestimmung der Thymidylat-Synthetase erlaubt Rückschlüsse auf therapeutische Wirkungen bei Behandlung chronischer Myelosen mit Cytosatica oder Röntgenbestrahlung der Milz und auf cytotoxische Nebenwirkungen während einer Carcinom-Behandlung mit 5-Fluor-Uracil. Auch die Kompensation eines Vitamin B12-Mangels bei perniziöser Anämie läßt sich an einem Abfall der Enzymaktivität im megaloblastären Knochenmark nachweisen.
