Zusammenfassung
Bei 13 Patienten mit akuten Leukämien und einem Patienten mit akutem Blastenschub bei chronischer Myelose werden als Parameter der DNS-Synthese in Leukämiezellen der3H-Thymidin-Markierungsindex und die Thymidin-Kinase unter Behandlung mit Prednisolon, Puri-Nethol, Methotrexat und Cytosin-Arabinosid untersucht. Zwischen beiden Größen besteht bei akuten Leukämien eine Korrelation, die bei den meisten Patienten auch unter einer cytostatischen Therapie erhalten bleibt. Bei einzelnen Patienten verhalten sich jedoch Thymidin-Kinase und Markierungsindex unabhängig voneinander. Dieses wird als spezifischer Effekt der Therapie auf die DNS-Synthese angesehen. Eine gleichzeitige und in etwa gleichmäßige Veränderung beider Parameter ohne paralleles Verhalten der Blasten wird als Hinweis für eine Erhöhung oder Erniedrigung des Anteils proliferierender Zellen gedeutet.
1. Die Verlaufsuntersuchungen unter Behandlung mit 6-Mercaptopurin (Puri-Nethol®) zeigen keine direkte Beeinflussung der DNS-Synthese.
2. Unter Behandlung mit Prednisolon ist in den empfindlichen Zellen — bevorzugt Lymphoblasten — ein stärkerer und rascherer Abfall der Thymdin-Kinase im Vergleich zum3H-Markierungsindex Hinweis für eine Einwirkung der Steroidbehandlung auf den Thymidin-Stoffwechsel.
3. Unter Methotrexat fällt bei Hemmung der FH2-Reduktase ein Anstieg, bei wiederansteigender FH2-Reduktase ein Abfall der Thymidin-Kinase auf. Dieses Verhalten der Thymidin-Kinase muß nicht mit dem des3H-Thymidin-Markierungsindex korrelieren. Es sind daher unter Methotrexat-Behandlung Aktivitätsänderungen der Thymidin-Kinase in den proliferierenden Zellen anzunehmen.
4. Ein Anstieg der Thymidin-Kinase läßt sich auch unter einer wirksamen Behandlung mit Cytosin-Arabinosid nachweisen. Hier kann die Enzymaktivität auf extrem hohe Werte ansteigen, wenn sie auf die mit3H-Thymidin markierten Zellen bezogen wird.
Die Veränderungen unter Behandlung mit Methotrexat und Cytosin-Arabinosid werden durch die bekannten biochemischen Angriffspunkte dieser Antimetabolite auf die DNS-Synthese erklärt [22, 24].
Summary
The3H-thymidine labelling index and the thymidine kinase is determined in leukemic cells of 13 patients with acute leukemia and 1 patient with acute blast crisis in the course of chronic myelocytic leukemia under treatment with Prednisolone, 6-Mercaptopurine, Methotrexate and Cytosine Arabinoside. In acute leukemias both parameters of DNA synthesis are correlated to each other, in most cases not only before but also under therapy. However in a few patients under therapy an independant change of thymidine kinase and of labelling index is observed. This is interpreted as a specific effect of therapy on DNA-synthesis. An equal change of both values at the same time is only to be evaluated as sign of increase or decrease of proliferating cells.
1. Under treatment with 6-Mercaptopurine no direct influence on DNA-synthesis is observed.
2. A more pronounced and more rapid decrease of thymidine kinase compared with the labelling index under treatment with Prednisolone is a sign of the effect of this treatment on thymidine metabolism in sensitive cells, preferable lymphoblasts.
3. Under treatment with Methotrexate the thymidine kinase is increased during the stage of FH2-reductase inhibition and decreased when the FH2-reductase rises to high values. This change of thymidine kinase must not correlate to the labelling index. It is assumed therefore a change of thymidine kinase activity in the proliferating cells.
4. An increase of thymidine kinase is also observed under treatment with Cytosine Arabinoside. The activity can reach extreme high values when based on the number of labelled cells.
The changes caused by treatment with Methotrexate and Cytosine Arabinoside are explained by biochemically defined mechanisms of action of these antimetabolites on DNA-synthesis [22, 24].
