Zusammenfassung
An verschiedenen Beispielen von Gasaustauschstörungen der Lunge wird die Ein-Atemzug-CO-Methode (D LCO) auf ihre differentialdiagnostische Aussagekraft geprüft und mit den arteriellen Blutgasanalysen in Ruhe und unter Belastung verglichen. Die an gesunden Spitalpersonen bestimmten Normwerte stimmen mit denen anderer Autoren überein. Da schwer dyspnoische Patienten nicht in der Lage sind, den Atem während 10 sec anzuhalten, untersuchten wir den Einfluß einer geringen Rückatmung. Die so modifiziert gemesseneD L CO weicht für die klinische Interpretation nur wenig von einer normal gemessenen ab. DieD L CO bewährt sich als Screening-Test zur Erfassung von Diffusionsstörungen, welche sonst zur Abklärung bedeutend aufwendigere Methoden fordern.
Summary
The measurement of pulmonary diffusing capacity by the single breath method (D L CO ) is tested on patients with various pulmonary diseases associated with troubles of gas exchange. The results are compared with arterial analysis of blood gas. The normal values are in agreement with those of other authors. The breathholding period is difficult for very dyspnoic patients. It can be eased by rebreathing. The results obtained by this modification differ only little from normal measurements. TheD L CO is proved as screening for diagnosis of pulmonary diffusing capacity diseases which are otherwise very difficult to characterise.
