Zusammenfassung
Es wird über einen Fall von Diabetes inspidus renalis bei einem weiblichen Säugling berichtet. Der Säugling kann den Harn nur bis maximal 80 mosm/kg H2O konzentrieren, der D.i.r. liegt also in voller Ausprägung vor. Der Vater des Säuglings leidet an einer intermediären Form des D.i.r. In der Diskussion wird eine Abgrenzung der intermediären Formen von den kompletten Formen des D.i.r. sowie von den Gesunden vorgeschlagen. Komplette Formen (=Homozygote bzw. Hemizygote) erreichen im Konzentrationsversuch höchstens einen Uosm/Posm-Quotienten von 1, die Harnosmolalität steigt maximal bis 295 mosm/kg H2O an. Die intermediären Formen (=Heterozygote) weisen einen maximalen Uosm/Posm-Quotienten zwischen 1 und 3 auf, ihre Harnosmolalität liegt maximal zwischen 295 und 850 mosm/kg H2O. Die kompletten Formen sprechen nicht auf exogenes Vasopressin an, die intermediären Formen des D.i.r. zeigen eine schwache bis mittelstarke Antidiurese nach Vasopressin.
