Zusammenfassung
Zum Nachweis möglicher Zusammenhänge der Reninaktivität im Plasma mit Veränderungen des Plasmavolumens und der Elektrolytausscheidung bei simulierter Hypervolämie durch Immersion wurden an 3 Versuchspersonen die Natrium-/Kaliumausscheidung, an 3 weiteren Versuchspersonen das Plasmavolumen und an 8 Probanden die Reninaktivität im Plasma nach einer einstündigen Liegeperiode und nach sechsstündiger Immersion gemessen.
Der Anstieg des Na/K-Quotienten um etwa das 2–3fache, der auf eine Verminderung der Aldosteronsekretion nach mehrstündiger Immersion zurückgeführt wird, verläuft parallel mit einer Abnahme der Reninaktivität um 28%.
Das Plasmavolumen nimmt um 14,1% ab. Eine reziproke Beziehung des Plasmavolumens zur Reninaktivität liegt bei Immersion nicht vor.
Es wird diskutiert, daß die bei Immersion frühzeitig zu beobachtende Erhöhung des tubulären Natrium-load zu einer Verminderung der Reninausschüttung führt, die ihrerseits eine erniedrigte Aldosteronsekretion induziert und so die Veränderungen der Elektrolyturinausscheidung auslöst.
Summary
During whole body immersion a relationship between plasma renin activity and changes in plasma volume and electrolyte excretion by the kidney was investigated.
In eleven healthy young men the following parameters were measured in a control period and after immersion in thermoindifferent water (34–34,5° C) for six hours: plasma renin activity (eight experiments), sodium- and potassium excretion (three experiments) and plasma volume (three experiments).
The significant decrease of plasma renin activity by 28% agrees with the increase of the sodium/potassium quotient, which may reflect a decreased aldosterone secretion.
Since plasma volume shows a reduction by 14,1%, an inverse relationship to plasma renin activity is not observed in our immersion experiments.
Judging from the sodium/potassium quotient the early natriuretic effect of immersion must be independent of aldosterone secretion.
The following hypothesis is proposed: the early increase in tubular sodium load is the main reason for the decreased renin activity. This decrease induces a reduction in aldosterone secretion, which is responsible for the changes in electrolyte excretion by the kidney after immersion for some hours.
