Summary
In the course of hemodialysis of patients with chronic renal diseases the binding of bromsulfophthalein to albumin which is detectable by acrylamide gel electrophoresis, is nearly inversely proportional to the concentration of free fatty acids. The free fatty acids displace the dye from its binding to albumin. This effect is produced in vivo by the temporarily increased concentration of fatty acids as a result of the lipolysis after heparinization which is necessary for baemodialysis. In vitro the effect can be detected after addition of oleic acid to an albumin—bromsulfophthalein solution. This displacement mechanism is effective already in hyperlipacidaemia. It is possible that due to the change of the albumin binding capacity after lipolysis certain drugs are separated from their albumin binding. Thereby the effect of the drug on the tissue may be changed and/or the excretion by the kidneys affected.
Zusammenfassung
Im Verlauf der Hämodialysc chronisch Nierenkranker verhält sich die mit der Acrylamidgel Elektrophorese nachweisbare Bindung von Bromsulphthalein und Albumin etwa umgekehrt proportional zur Konzentration „unveresterter Fettsäuren“. Diese Fettsäuren verdrängen den Farbstoff aus seiner Albuminbindung. Der Effekt ist in vivo durch die vorübergehende Zunahme von Fettsäuren infolge der Postheparin-Lipolyse anläßlich der Heparinisierung bei der Hämodialyse auslösbar und auch nach Zugabe von Ölsäure zu einer reinen Albumin-Bromsulphthalein-Lösung in vitro nachweisbar. Bereits bei physiologischen Hyperlipacidämien wird dieser Verdrängungsmechanismus wirksam. Er kann möglicherweise — dem Bromsulphthalein vergleichbar — Medikamente aus ihrer Albuminbindung verdrängen, wodurch deren Wirkung auf die Gewebe und/oder die Ausscheidung durch die Nieren beeinflußt wird.
