Nierentransplantation bei hämolytisch-urämischem Syndrom

Kidney transplantation in haemolytic-uraemic syndrome

Summary

The clinical course of the case of a 14 year old girl, who after a minor “viral” infection, suffered from severe haemolytic-uraemic syndrome (Gasser) with irreversible renal insufficiency is subject of this report. Seven months after the acute phase of the disease (characterised by haemolytic anaemia, cerebral seizures, oliguria and hypertension) a cadaver kidney was transplanted. In contrast to most reports, the patient's own kidneys removed at the time of transplantation did not show histological signs of renal cortical necrosis but those of malignant nephrosclerosis. The postoperative course, aside from some urological problems and one episode of acute graft rejection, was uneventful. During the two and half year observation period since then no signs of the original disease process have reocurred.

Zusammenfassung

Gegenstand des Berichtes ist der Krankheitsverlauf einer Patientin, bei der sich im Alter von 14 Jahren nach einem „Virusinfekt“ ein schweres hämolytisch-urämisches Syndrom (Gasser) entwickelte. Auch nach Überwindung der akuten Krankheitsphase mit Hämolyse, Oligurie, Krampfanfällen und Hypertonie blieb die Patientin niereninsuffizient und wurde über 7 Monate hämodialysiert. Dann gelang die Transplantation einer Leichenniere. Die dabei entfernten Nieren der Patientin zeigten zu diesem Zeitpunkt im Unterschied zu den meisten Berichten histologisch keine Nierenrindennekrose, sondern eine maligne Nephrosklerose. Der postoperative Verlauf war nach anfänglichen chirurgisch-urologischen Komplikationen und einer akuten Abstoßungskrise ungestört. Während einer jetzt 2 1/2jährigen Beobachtungszeit ist ein Rezidiv der Grundkrankheit klinisch nicht wieder manifest geworden.
