Zusammenfassung
Es wurde die fäkale Lipidausscheidung sowie die Fettsäuren- und Sterinzusammensetzung der einzelnen Lipidfraktionen von 27 Gesunden und von 40 Cirrhosekranken mittels kombinierter Dünnschicht- und Gaschromatographie untersucht. Dabei fanden sich starke Veränderungen, insbesondere eine signifikante Vermehrung der Gesamtlipidexkretion und absolute Zunahme der freien Fettsäuren neben einer etwas geringeren Vermehrung der Sterinester, bei den Cirrhosen mit Zeichen epithelialer oder/und portaler Dekompensation. Bei kompensierten Cirrhosen bestehen keine oder nur geringe Veränderungen. In einzelnen Fällen ist als Zeichen einer geringen oder beginnenden Fettverdauungsinsuffizienz die Verschiebung der Relation der Fraktionen zueinander bereits vorhanden, also die freien Fettsäuren vermehrt, während die Gesamtlipidexkretion noch nicht gesteigert ist. Bei den gaschromatographischen Untersuchungen sind einerseits Änderungen der Triglycerid-Fettsäuren bei dekompensierten Cirrhosen im Sinn einer Zunahme der Ölsäure festzustellen. Andererseits zeigen sich Änderungen des Sterinmusters, wobei die Hemmung des bakteriellen Umbaus von Cholesterin zu Koprosterin bei Cirrhotikern, und zwar offenbar in Abhängigkeit vom Schweregrad der Erkrankung, nachzuweisen ist.
Die Veränderungen werden auf die Störung der Gallensäurensynthese im Rahmen der Leberparenchymschädigung und die hierdurch verursachte Resorptionsstörung infolge des Gallensäuremangels zurückgeführt. Die Änderungen des Sterinmusters haben ihre Ursache in einer Änderung des Darmbakterienflors.
Summary
Fecal lipid excretion and fatty acid and sterol composition of the lipid fractions were examined in 27 healthy subjects and 40 patients suffering from liver cirrhosis. Patients with decompensated liver cirrhosis showed a significant increase in total lipid excretion as well as an relative and absolute augmentation of free fatty acids and sterol esters in stool. No significant changes were seen in compensated cirrhosis. In some cases excretion of free fatty acids is relatively augmentated, whereas total lipids remain unchanged. This is thought to be due to a disturbance in fat digestion. Gas chromatographic analyses showed an augmentation of oleic acid within the triglyceride fraction in patients with decompensated cirrhosis. On the other hand, the sterol composition of lipid fractions is changed depending on the state of liver cirrhosis an inhibition of bacterial cholesterol metabolism to coprosterol could be shown.
These changes are referred to disturbances in the synthesis of bile acids leading to a decreased reabsorption of lipids. The changes in sterol composition of stool are due to an altered intestinal bacterial flores.
