Zusammenfassung
1. Die Ampicillin-Clearance wurde bei 15 Patienten mit Glomerulumfiltratwerten zwischen 19 und 174 ml/min gemessen und mit den simultan bestimmten Inulin- und PAH-Clearances verglichen. Beim Normalen betrug die Ampillicin-Clearance im Durchschnitt 240 ml/min. Im ganzen Kollektiv fand sich eine lineare Korrelation sowohl zwischen Ampillicin- und Inulin-Clearance als auch zwischen Ampillicin- und PAH-Clearance. Der CAmp/C In-Quotient betrug im Durchschnitt 2,01, derC Amp/C PAH-Quotient 0,403. Letzterer wies keine Abhängigkeit von den Ampicillin-Plasmakonzentrationen zwischen 2,1 und 65,7 µg/ml auf.
2. Der Abfall des Ampicillin-Plasmaspiegels nach einmaliger intravenöser Injektion wurde bei 31 nierengesunden und nierenkranken Patienten (darunter 7 mit Anurie) über mehrere Stunden verfolgt. Der Ampicillin-Eliminationskoeffizientk stand in linearer Korrelation zur Inulin-Clearance (k=0,115+0,00503C In) und zur PAH-Clearance (k=0,131+0,000917C PAH). Der Wertk = 0,115 bzw.k=0,131 dürfte der extrarenalen Ausscheidung allein entsprechen mit einer mittleren Halbwertszeit von 5,6 Std. Bei Nierengesunden betrugk durchschnittlich 0,73, entsprechend einer Halbwertszeit von 57 min, und der renale Anteil der Ausscheidung lag um 84%. Das Verteilungs-volumen des Ampicillins betrug durchschnittlich 39% des Körpergewichts. Bei 6 Patienten mit Anurie erhöhte sichk während der Hämodialyse im Durchschnitt von 0,077 auf 0,171.
3. Bei 3 Patienten bewirkte die intravenöse Verabreichung von 1 g Ampicillin eine 25%ige Abnahme der PAH-Clearance. Dagegen wurde bei 2 Patienten die PAH-Extraktion dadurch nicht beeinflußt, was für eine vasomotorische Reaktion spricht.
4. Bei 7 Patienten wurden die Plasmaspiegel nach peroraler und intravenöser Verabreichung verglichen. Die enterale Resorption lag im Durchschnitt um 54% der eingenommenen Menge.
5. Es wird die adäquate Dosierung des Ampicillins bei eingeschränkter Nierenfunktion festgelegt.
Summary
1. The Ampicillin-clearance was measured in 15 patients with glomerular filtration rates between 19 and 174 ml/min and compared with the inulin and PAH clearances. The Ampicillin/inulin clearance ratio averaged 2.01, the Ampicillin/PAH clearance ratio 0.403. These ratios were independent of the Ampicillin plasma concentration and remained practically constant at any clearance level.
2. The behaviour of the Ampicillin plasmal levels after a single intravenous injection was investigated in 31 patients with normal or reduced renal function (7 subjects with anuria). After equilibration the decrease was exponential. The elimination coefficient of Ampicillink showed a linear correlation with the inulin and the PAH clearance as well (k=0.115+0.00503C In andk=0.131+0.000917C PAH). The extrarenal elimination could be expressed ask=0.115 ork=0.131, corresponding to a half-life of 5.6 hours. In normal subjectsk averaged 0.73, the half-life was 57 minutes, and the renal fraction of the total elimination was 84%. The volume of distribution of Ampicillin averaged for the whole series 39% of the body weight. In 6 anuric patientsk rose from a mean value of 0.077 to 0.171 during hemodialysis.
3. A single intravenous injection of 1 g of Ampicillin produced an acute reduction inC PAH averaging 25%, but no decrease in the PAH extraction ratio.
4. In 7 patients the Ampicillin plasma levels were compared after oral and intravenous administration. The enteral absorption of the drug averaged 54% of the intake.
5. An adequate dosage of Ampicillin in renal disease has been calculated from these data.
