Zusammenfassung und Schlußfolgerungen
Bei Kindern mit Diabetes insipidus renalis wurden insgesamt achtmal Vasopressin-Teste unter Clearance-Bedingungen durchgeführt und die Veränderungen der Wasser- und Elektrolyt-Ausscheidungen bestimmt. Die Kinder standen unter salzarmer Kost und erhielten entweder keine Medikamente oder kurzfristig oder langdauernd Esidrix®. Das Vasopressin bewirkte eine prompte Erhöhung der Frei-Wasser-Clearance und des Harnvolumens und einen verzögerten Anstieg der Natrium-Clearance. Diese beiden Effekte werden so gedeutet, daß das Vasopressin eine Stimulation der Natrium-Rückresorption im aufsteigenden Schenkel der Henleschen Schleife bewirkt. Die Natriurese wird als eine sekundäre Folge aufgefaßt, die durch die Erhöhung des Natrium-Gradienten zwischen interstitieller Flüssigkeit des Nierenmarks und dem Sammelrohrharn auftritt. Es wird vermutet, daß die beiden physiologischen Wirkungen des ADH — die antidiuretische und die Na-aktive — an zwei verschiedenen Stellen des Nephrons angreifen und synergistisch eine maximale Konzentrierung des Harnes herbeiführen.
Summary
Eight vasopressin-tests were performed in 3 children with nephrogenic diabetes insipidus and the changes in the clearances of inulin, p-aminohippuric acid, sodium, potassium, chloride and free-water and of the urinary output were determined simultaneously. The children were on a low-salt diet and had either no treatment or received hydrochlorothiazide for 4 days or a longer period. Vasopressin (Pitressin s.c. or lysin-vasopressin i.v.) produced a prompt increase of the free-water-clearance and the urinary flow and a delayed increase of the sodium-clearance. It is suggested that vasopressin acts in stimulating the sodium reabsorption in the ascending limp ofHenle's loop to elevate the sodium concentration in the renal papilla. The natriuresis is thought to be a phenomenon secondary to the elevation of the sodium gradient between medullary interstitial fluid and the collecting duct urine. It is suggested that both effects of vasopressin—the antidiuretic and the “natriuretic”—act synergistically at two different segments of the nephron to produce a maximally concentrated urine.
