Summary
The influence of the increased cardiac output on the arterial blood pressure in early essential hypertension is not yet fully understood. Investigations have been undertaken to examine the hemodynamic effects of diminution of cardiac output byβ-receptor blockade. 25 hypertensive patients, 18 with early and 7 with advanced hypertension were studied in acute experiments. In addition, 5 patients with early essential hypertension were treated for 4 to 12 months by oral Propranolol administration.—The control values for the cardiac index and heart rate were increased in patients with early essential hypertension if the mean arterial pressure was raised. The following alterations were observed in acute and chronic experiments under the effect of Propranolol:

The cardiac index was diminished by 5–45% in comparison to the control values. The changes were mainly caused by a decrease of the heart rate. There was a correlation between the extent of that decrease and the control value for the cardiac index.

Mean arterial pressure did not change significantly. However, in patients with early essential hypertension there was a tendency to an increased pressure, whereas a decrease of blood pressure was more typical in advanced hypertension.

The central venous pressure and the peripheral resistance were generally increased.
We interpret our data as follows: In the early stage of essential hypertension where the blood pressure is still oscillating between high and normal values, increase of the arterial pressure is not dependent on an increase of cardiac output. Accordingly, the alterations of cardiac output in the early stage are rather more highly compensated by an adaptation of the peripheral resistance than in advanced hypertension.
Zusammenfassung
Ausgehend von der Frage, welchen Einfluß das bei beginnender essentieller Hypertonic erhöhte Herzzeitvolumen auf die Höhe des arteriellen Blutdrucks ausübt, wurden die kreislaufmechanischen Auswirkungen einer Verminderung des Herzzeitvolumens durch Betareceptorenblockade geprüft. Akute Versuche wurden bei 25 Hypertonikern, davon 18 Patienten mit beginnender und 7 Patienten mit fortgeschrittener Hypertonie, vorgenommen. Zusätzlich wurden 5 Patienten mit beginnender essentieller Hypertonie über 4–12 Monate mit Propranolol behandelt.
Auch in dieser Untersuchung waren die Ausgangswerte für Herzindex und Herzfrequenz bei Patienten mit beginnender essentieller Hypertonie gesteigert, sofern der arterielle Mitteldruck erhöht war. Unter Propranolol wurden im akuten und chronischen Versuch folgende Veränderungen beobachtet:

Überwiegend infolge Abnahme der Herzfrequenz wurde das Herzzeitvolumen um 5–45% des Ausgangswertes vermindert. Das Ausmaß der Verminderung zeigt eine Korrelation zur Höhe des Herzzeitvolumens im Ausgang.

Der arterielle Mitteldruck wurde nicht signifikant verändert. Bei beginnender essentieller Hypertonie trat jedoch eher ein Blutdruckanstieg, bei fortgeschrittener Hypertonie eher ein Blutdruckabfall auf. Eine Korrelation zur Abnahme des Herzzeitvolumens bestand nicht.

Zentraler Venendruck und peripherer Widerstand stiegen generell an.
Die Befunde wurden so interpretiert, daß in der beginnenden Phase der essentiellen Hypertonie, in der der arterielle Mitteldruck noch zwischen erhöhten und normalen Werten schwankt, die Blutdrucksteigerung nicht durch die Zunahme des Herzzeitvolumens verursacht wird. Offenbar werden Veränderungen des Herzzeitvolumens bei beginnender Hypertonie stärker, bei fortgeschrittener Hypertonie weniger durch eine Anpassung des Gefäßwiderstandes ausgeglichen.
