Prostaglandins and their clinical application

Summary

A short chronological review is given of the discovery, isolation, chemical structure and biosynthesis of prostaglandins, as well as a description of their clinical applications. Recently, the prostaglandins have been quite frequently studied as uterine stimulant in the women's hospitals where they are used both in women at full term and in the early stages of pregnancy. In the latter case, the prostaglandins induce abortion. They possibly also influence the fertility of both man and woman.

A more extensive clinical investigation on the use as bronchial antispasmodic seems desirable.

On the basis of data obtained in animal experiments suggestions are made for the clinical use of prostaglandins in diabetes mellitus, hypertension, prevention of thrombotic processes, diseases of the blood vessels, irregular cardiac rhythm. In some of these cases a linoleic acid-rich diet, through increased endogenous prostaglandin synthesis, might be of value.

Zusammenfassung

Es wird eine kurze chronologische Übersicht über die Entdeckung, Isolierung, chemische Struktur und Biosynthese von Prostaglandinen gegeben und klinische Anwendungen werden beschrieben. Am häufigsten finden die Prostaglandine Anwendung als Uterusstimulans in der Frauenklinik, wo sie sowohl nach normaler Schwangerschaftszeit wie auch in früheren Stadien der Gravidität angewandt werden. Im letztgenannten Fall führen die Prostaglandine zu einem Abortus. Außerdem dürften die Prostaglandine eine Möglichkeit zur Beeinflussung der Fertilität beim Mann wie bei der Frau bieten.

Eine nähere Untersuchung der Anwendung von Prostaglandinen als Broncholytikum bei Bronchialasthma erscheint wünschenswert.

An Hand von tierexperimentellen Daten werden Beispiele für die mögliche Anwendung von Prostaglandinen gegeben, und zwar bei Diabetes mellitus, Hypertonie, zur Vorbeugung thrombotischer Prozesse, bei Gefäßerkrankungen und Herzrhythmusstörungen. In einigen dieser Fälle könnte vielleicht eine Diät mit einem hohen Gehalt an Linolsäure durch eine erhöhte endogene Prostaglandinsynthese wertvoll sein.
