Zusammenfassung
Von 17 Patienten mit allogenen Nierentransplantaten wurden 115 Urinkonzentrate in verschiedenen Zeitintervallen bis zu 1 1/2 Jahren nach der Transplantation immunologisch mit 6 verschiedenen Antiseren untersucht. Gegenüber Normalbefunden der Proteinurie fand sich während Abstoßungskrisen eine quantitativ und qualitativ veränderte Histurie (Ausscheidung von Gewebsantigenen), eine vermehrte Ausscheidung eines γ-Globulinbruchstücks von α2-Mobilität und eine Ausscheidung von Fibrinogenspaltprodukten im Harn nierentransplantierter Patienten. Bei den Befunden der veränderten Histurie ergab sich in 92,8% der Fälle, bei denen der Fibrinogenspaltprodukte in 78,5% und bei der vermehrten Ausscheidung des γ-α2-Globulinbruchstücks in 70,5% eine positive Korrelation zum klinischen Verlauf. Auf die Bedeutung dieser Befunde zur Sicherung der Diagnose einer Abstoßungsreaktion und eine mögliche Erklärung dieser Befunde wird eingegangen.
Summary
In 17 kidney grafted patients 115 urine-concentrates were immunologically analysed with 6 different antisera at different time intervals up to 1 1/2 years after grafting. In contrast to normal proteinuria during rejection crises a quantitative and qualitative change of histuria (excretion of tissue antigens), an increased excretion of one γ globulin fragment of α2 mobility and the excretion of fibrinogen split products were found in urineconcentrates of kidney grafted patients. In regard to the variations of histuria a positive correlation to the clinical course was found in 92.8%; in regard to fibrinogen split products in 78.5% and to the increased excretion of the γ-α2 fragment in 70.5%. The diagnostic value of these findings in correlation to rejection crises and the possible explanation of these findings is discussed.
