Summary
The influence of a single intravenous injection of heptaminol (10 mg/kg) or aminophylline (0.48 g) on the pulmonary circulation and right ventricular contractile function was studied in 10 patients with chronic cor pulmonale. The following results were observed:

Heptaminol produced a significant rise of heart rate and systemic arterial pressure. There was also a slight increase of cardiac output and right ventriculardp/dt max. Pulmonary arterial pressure, pulmonary arterial wedge pressure, pulmonary vascular resistance, and right ventricular enddiastolic pressure did not show any consistent changes in patients without pulmonary hypertension. Patients with pulmonary hypertension showed a slight additional rise in pulmonary arterial pressure and pulmonary vascular resistance.

In contrast to heptaminol, aminophylline produced a significant decrease of pressure and resistance in the systemic and pulmonary circulation. The hemodynamic effect on pulmonary circulation was more pronounced in patients with pulmonary hypertension. Cardiac output remained unchanged. There was a small but uniform fall of enddiastolic right ventricular pressure and pulmonary arterial wedge pressure following aminophylline. In comparison to heptaminol, the increase of heart rate and right ventriculardp/dt max was more evident.
Zusammenfassung
Bei 10 Patienten mit chronischem Cor pulmonale wurde der Einfluß einer intravenösen Injektion 1. von 10 mg/kg Heptaminol, 2. von 0,48 g Euphyllin® auf den Lungenkreislauf und die contractile Funktion des rechten Ventrikels untersucht.
Dabei wurden folgende Ergebnisse gewonnen:

Heptaminol bewirkte einen Anstieg von Herzfrequenz und arteriellem Druck. Auch für das Herzzeitvolumen sowiedp/dt max des rechten Ventrikels ergab sich eine ansteigende Tendenz, die jedoch außerhalb der Signifikanzschranken lag. Dagegen ließen sich gerichtete Änderungen von Pulmonalarteriendruck, Pulmonalcapillardruck, Lungengefäßwiderstand und enddiastolischem rechten Ventrikeldruck nicht nachweisen. Bei Unterteilung des Untersuchungsgutes in Patienten mit und ohne pulmonale Hypertonie war sogar eine leichte Zunahme der Druck- und Widerstandsgrößen im kleinen Kreislauf bei den Patienten mit pulmonalem Hochdruck auffällig.

Euphyllin® führte demgegenüber zu einer deutlichen Druck- und Widerstandsabnahme sowohl im großen wie im kleinen Kreislauf. Der hämodynamische Effekt auf den Lungenkreislauf war bei den Patienten mit pulmonaler Hypertonie ausgeprägter als bei den Patienten, die einen noch normalen Ausgangswert für PA-Druck und Lungengefäßwiderstand aufwiesen. Das Herzzeitvolumen blieb im Mittel unverändert. Enddiastolischer rechter Ventrikeldruck und linksventriculärer Füllungsdruck zeigten fast ausnahmslos einen geringgradigen, wenngleich gerichteten Abfall. Die Steigerung von Herzfrequenz unddp/dt max des rechten Ventrikels war durchschnittlich ausgeprägter als nach Heptaminol.
Die möglichen Ursachen der Drucksenkung im Lungenkreislauf nach Euphyllin® werden diskutiert.
