Zusammenfassung
Bei 23 Patienten mit hepatogenem Ascites und 2 Patienten mit nephrotischem Syndrom wurde die Wirkung von 200–600 mg Spirolacton (SC 9420) täglich per os auf die renale Kationenund Wasserausscheidung untersucht. Eine vollständige Entwässerung des Ödems konnte in 17 Fällen durch Spirolacton allein, bei weiteren 4 Patienten erst durch Kombination mit Hydrochlorothiacid bzw. Triamcinolon und isotonische Humanalbumininfusionen erzielt werden. In 4 Fällen gelang die Ödembeseitigung weder mit Spirolacton noch durch zusätzliche Entwässerungsmaßnahmen. Indikation und Verlauf einer derartigen Behandlung sowie die möglichen Ursachen des Versagens werden diskutiert.
