Zusammenfassung
Drei Patienten mit therapierefraktärem Ascites bei Lebercirrhose und zwei Patienten mit therapierefraktären Ödemen bei Rechtsinsuffizienz des Herzens wurden mit SU 4885 (Metopiron), einem Blocker der 11-β-Hydroxylierung in der NNR, unter Zugabe von Dexamethason oder Prednison und Saliuretica behandelt. Während unter SU 4885 und Dexamethasongaben alleine bei den drei Patienten mit therapierefraktärem Ascites kein diuretischer Effekt erzielt werden konnte, setzte nach Zugabe eines Saliureticums jeweils eine Natriurese und Wasserdiurese mit Gewichtsabnahme und vollständiger oder teilweiser Ausschwemmung des Ascites ein. Bei den beiden Patienten mit therapierefraktären kardialen Ödemen war kein Einfluß der Kombinationsbehandlung zu sehen. Die erhöhte Aldosteronausscheidung im Urin sank unter Verabreichung von SU 4885 auf teilweise nicht mehr meßbare Werte ab, ebenso die Cortisol- und Cortisonausscheidung. Bei letzterer ist allerdings der Bremseffekt des Dexamethasons mit zu berücksichtigen.
Als Erklärung für das unterschiedliche Ansprechen von hepatogenem Ascites und kardialen ödemen auf die kombinierte Metopiron-Dexamethason-Esidrix-Behandlung wird die unterschiedliche Bedeutung der Mineralcorticoide bei den verschiedenen Ödemarten diskutiert. Die Annahme, daß bei dekompensierten Leberkranken eine ungenügende Inaktivierung der Mineralcorticoide in der Leber für die Na- und Wasserretention von größerer Bedeutung ist als bei dekompensierten Herzkranken, ist anscheinend gerechtfertigt. Die Tatsache jedoch, daß die Beseitigung der Salz- und Wasserretention meist erst nach Zugabe eines vorher unwirksamen Saliureticums gelingt, spricht dafür, daß eine zusätzliche Blockierung der vermehrten Na-Rückresorption im proximalen Tubulusabschnitt für einen Therapieerfolg maßgebend ist.
