Zusammenfassung
Vergleichende Bestimmungen des endogenen Kreatinin in Serum und Harn mit der Methode vonPopper, Mandel u.Mayer (1937) und derjenigen vonBonsnes u.Taussky (1945) in der Modifikation vonBrod u.Sirota (1948) ergaben, daß die Methode vonPopper vorzuziehen ist. Hierfür werden methodische Gründe dargelegt. Außerdem wird gezeigt, daß mit dieser Bestimmung die sog. Nicht-Kreatinin-Chromogene nur noch gering erfaßt werden.
Bei 127 Patienten (69 Männer, 58 Frauen) wurde das Glomerulusfiltrat mit der endogenen Kreatininclearance und bei je 15 Männern und Frauen mit der simultan bestimmten Clearance von Inulin und Kreatinin normal gefunden.
In den nach Männern und Frauen getrennten Altersgruppen von je 10 Jahren wird der mittlere Normwert des Serumkreatinin, dessen Meßbereich und der Normalbereich angegeben. Für Männer zwischen 20 und 50 Jahren beträgt der mittlere Normwert 0,92 mg-% und für Frauen 0,76 mg-%. In den anderen Altersgruppen liegen die Werte etwas niedriger. Diese mit der als „unspezifisch“ geltenden Methode vonPopper u. Mitarb. gefundenen Werte stimmen sehr genau mit den in der Literatur angegebenen Werten „spezifischer“ Methoden überein, welche nur das „wahre“ Kreatinin erfassen, aber kompliziert sind.
Das Verfahren vonPopper ist einfach, dennoch genau in der Bestimmung des endogenen Kreatinin und daher besonders für Routineuntersuchungen geeignet. Aceton und Acetessigsäure aber geben falsch hohe Werte.
