Simultaneous recording of monophasic action potentials and contractile force from the human heart

Zusammenfassung

Monophasische Aktionspotentiale (MAP) wurden simultan mit dem Kontraktilitätsparameter LVdp/dt max vom menschlichen Herzen während rechtsatrialer Stimulation aufgezeichnet. Dabei gestattete eine neuentwickelte, nicht auf dem Saugprinzip beruhende Ableittechnik die kontinuierliche Langzeitregistrierung der MAP vom Endomyokard. Nach sprunghaften Änderungen der Herzfrequenz zeigen MAP-Dauer und Kontraktilitätsgröße zunächst eine schnelle, nachfolgend eine langsame Phase der Adaptation an den der neuen Frequenz entsprechenden Gleichgewichtswert. Nach Erhöhen der Herzfrequenz scheint die langsame Phase der MAP-Verkürzung in enger Beziehung zur langsamen Kontraktilitätszunahme zu stehen, während nach sprunghafter Rückschaltung auf die Ausgangsfrequenz die langsame Phase der MAP-Verlängerung nicht von Kontraktilitätsänderungen begleitet wird. Die Übereinstimmung dieser Befunde mit solchen aus entsprechenden in-vitro Untersuchungen läßt dieses methodische Vorgehen als geeignet erscheinen, Vorgänge der elektromechanischen Kopplung auch am menschlichen Herzen zu untersuchen.

Summary

Cardiac monophasic action potentials (MAPs) and contractility have been simultaneously measured in man while the heart rate was being changed by right atrial pacing. A new non-suction electrode was used for safe and long-term recording of MAPs from the endomyocardium. Abrupt changes in cycle-length were followed first by a fast, then a slow response in the adaptation of MAP-duration and of contractility (LVdp/dt max) to the new steady state. Afterincreasing the heart rate the slow phase of MAP shortening appears to be related to the slow staircase of contractility, whereas after the stepdecrease of frequency no such a relation could be observed. The consistency of these results with those obtained in corresponding in-vitro experiments indicates that this methodical approach may be suitable for assessing the process of E-C coupling in the human heart.
