Zusammenfassung
Time motion Echokardiogramme (ECHO) von 30 Patienten mit Vorhofseptumdefekt (ASD) und 36 Personen ohne Herzerkrankung (C) wurden vergleichend ausgewertet. Es war das Ziel der vorliegenden Untersuchungen, echokardiographische Parameter festzustellen, die die Größe des Links-Rechts-Shunts (LRS) anzeigen. Die Diagnose des Herzfehlers und die Größe des Shunts wurden durch Herzkatheteruntersuchung gesichert; bei den Patienten mit ASD lag der Quotient aus pulmonalem und systemischem Zeitvolumen,Q p:Q s, zwischen 1,33 und 3,85:1; der pulmonale Widerstand war nicht erhöht. Der Quotient des links- (LVD) und rechtsventrikulären Durchmessers (RVD), LVD: RVD, und der RVD-Index waren bei Patienten mit ASD signifikant von den Werten der C verschieden: 1,07±0,04 vs. 3,1±0,03 und 22,5±0,5 vs. 9,8±0,2 mm/m2 (Mittelwerte ±1 SEM),p≦0,001. Die Beweglichkeit des interventrikulären Septums war bei 84% der Patienten mit ASD abnormal und korrelierte, wie die anderen geprüften Einzelparameter, nicht mit der Größe des LRS. Der QuotientQ p:Q s zeigte jedoch im untersuchten Bereich zwischen 1,33 und 3,85:1 eine lineare Korrelation mit dem Quotienten, der aus der maximalen diastolischen Öffnung der Tricuspidal- (TVO) und Mitralklappe (MVO), TVO/MVO, gebildet wurde, mit einem Koeffizientenr=0,76,p < 0,001. Bei jedem Patienten mit ASD und einem QuotientenQ p:Q s≧1,4:1 war der Quotient TVO/MVO > 1,15, während bei den C TVO/MVO zwischen 0,7–1,13 lag mit einem Mittelwert von 1,02±0,01. Die vorliegenden Daten sprechen dafür, daß mit der echokardiographischen Untersuchungstechnik bei Patienten mit ASD ein signifikanter LRS näherungsweise quantitativ erfaßt werden kann, und daß diese Patienten von jenen mit ASD und kleinem LRS und/oder Herzgesunden differenziert werden können.
Summary
Time motion echocardiograms (ECHO) of 30 patients with atrial septal defect (ASD) and 36 individuals without heart disease (C) were comparatively studied to see whether parameters may be defined indicating the size of the left-to-right shunt (LRS). The diagnosis was proven by cardiac catheterization; in the patients with ASD the pulmonic-to-systemic blood flow ratios,Q p:Q s, ranged from 1.33 to 3.85:1, and pulmonary vascular resistance was normal. The ratio of the left (LVD) and right ventricular dimension (RVD), LVD:RVD, and the RVD indices of the patients with ASD were significantly different from those of the C:1.07±0.04 vs. 3.1±0.03 and 22.5±0.5 vs. 9.8±0.2 mm/m2 respectively (mean ±1 SEM),p≦0.001. These parameters and the abnormal interventricular septal motion in 84% of patients with ASD did not correlate with the size of the LRS. However, the ratioQ p:Q s between 1.33 and 3.85:1 linearly correlated with the ratio derived from the maximal diastolic opening of the tricuspid (TVO) and mitral valve (MVO), TVO:MVO, with a coefficient ofr=0.76,p < 0.001. In each patient with ASD and aQ p:Q s ratio ≧ 1.4:1 the ratio TVO:MVO was > 1.15, while in the C TVO:MVO ranged from 0.7–1.13 with a mean of 1.02±0.01 (1 SEM). The data suggest that in patients with ASD a significant left-to-right shunt may be quantified by ECHO and that these patients may be separated either from patients with ASD and a small LRS and/or from normal subjects.
