Die Bedeutung der Acidose bei der Carboanhydrase-Hemmungsdiurese

Zusammenfassung

In klinischen Versuchen an Gesunden und hydropischen Patienten wird die Abhängigkeit der CAH-Hemmungsdiurese vom Säure-Basengleichgewicht experimentell dargestellt. Unter metabolischer Acidose versagt die CAH-Hemmung. Dieses Versagen ist unabhängig vom Na-Bestand des Körpers. Kompensiert man die Basenverluste durch Kaliumbicarbonat, so kann die Diamoxdiurese fortlaufend aufrechterhalten werden. In ausgewählten Beispielen wird die klinische Problematik bei der Anwendung der mineralwirksamen Diuretica diskutiert.
