Clinical and diagnostical aspects of acute cytomegalovirus infections in previously healthy adults

Summary

The clinical symptoms of floride Cytomegalovirus infections of 18 previously healthly adults are reported. Fever was generally observed. In most cases myocarditis or hepatitis could be identified. Two of our patients died of on severe myocarditis. In 3 patients Guillain-Barré-syndrome was observed. Only in few of our cases the clinical picture was characterized by mononucleosis with systemic swelling of lymphnodes and thrombocytopenia or by pneumonia. Other symptoms were found quite seldom. The diagnosis of cytomegalovirus infection could be ascertained by:

  1. 1.

    detection of cytomegalovirus IgM antibody titers of >1:64,

  2. 2.

    a four fold increase in the titer of complement fixing antibodies,

  3. 3.

    Virus isolation from urine.

Zusammenfassung

Es wird anhand von 19 gesicherten Fällen über das klinische Bild der akuten Cytomegalievirus-Infektion bei zuvor gesunden Erwachsenen berichtet. Fieber ist die Regel. Die Erkrankung verläuft am häufigsten als leichte Myokarditis oder Hepatitis. 2 Kranke verstarben an den Folgen einer Myokarditis. Bei 3 Patienten fand sich eine Polyradikulitis (Guillain-Barré-Syndrom). Neben dieser Symptomatik kann ein Mononukleose-artiges Krankheitsbild mit Lymphomen, Thrombo- und Leukopenie oder eine Bronchopneumonie das Bild bestimmen. Andere Symptome sind selten. Die Diagnose kann durch folgende Kriterien gesichert werden:

    Nachweis eines Cytomegalie-IgM-Antikörpertiters von über 1:64,

  2. 2.

    Anstieg der Komplement-Bindungs-Reaktion um den Faktor 4,

  3. 3.

    Virusnachweis im Urin.
