Summary

The diffuse interstitial fibrosis of the lungs is due to a proliferation of interalveolar and interlobular connective tissue. The chest roentgenogram is the clue to a correct diagnosis. Typical radiologic signs are: a nodular, a reticular or a mixed reticulo-nodular pattern. The diffuse fibrosis gives rise to a diminution of the diffusing capacity and compliance of the lungs. Serologic methods are available for the diagnosis of farmer's lung and bird fancier's disease. For the rest, biopsies of lymph nodes and pulmonary tissue are necessary very often. For the diagnosis of diffuse interstitial fibrosis, we prefer the open biopsy by a little thoracocentesis.

Zusammenfassung

Die diffuse Lungenfibrose entsteht durch eine Vermehrung des interalveolären und interlobulären Bindegewebes. Der Weg zur Diagnose führt über das Thoraxröntgenbild. Typisch sind körnige, netzförmige und vor allem gemischte retikulonoduläre Lungenveränderungen. Bei der Lungenfunktionsprüfung steht eine Verminderung der Lungendehnbarkeit (Compliance) und der Diffusionskapazität im Vordergrund. Bei der Suche nach der Ätiologie einer Lungenfibrose stehen außer der Röntgendiagnostik serologische Methoden für die Farmerlunge und die Vogelzüchterkrankheit sowie bioptische Methoden zur Verfügung. Die idiopathischen Lungenfibrosen lassen sich gewöhnlich nur durch Lungenbiopsie sicher differenzieren.
