Summary
A “hyponatremic hypertensive crisis” was observed in four patients with renal insufficiency. In these patients sodium administration was followed by a reduction of blood pressure.
In two of the patients treatment with furosemide had elicited the hyponatremic hypertensive crisis. In the other two patients pressure-dependant natriuresis with malignant hypertension and haemodialysis with low sodium dialysate were the cause of severe hyponatremia.
The conclusion from these clinical observations is that in patients with renal insufficiency treatment of hypertension by forced reduction of body sodium is not always indicated, especially not if interstitial nephritis was the primary renal disease.
Zusammenfassung
Eine „hyponatriämisch-hypertone Krise“ wurde bei 4 Patienten mit Niereninsuffizienz beobachtet. Kochsalzzufuhr hatte bei diesen Patienten eine Blutdrucksenkung zur Folge.
Als auslösende Ursache der hyponatriämischen Hochdruckkrise muß bei zwei Fällen eine Behandlung mit Furosemid angenommen werden. Bei den anderen beiden Patienten war eine druckabhängige Natriurese im Rahmen einer malignen Hypertonie und eine Dialyse mit niedriger Natrium-Konzentration in der Spülflüssigkeit für die ausgeprägte Hyponatriämie verantwortlich.
Aus diesen klinischen Beobachtungen wird die Schlußfolgerung gezogen, daß eine forcierte Natriumverarmung bei Patienten mit Niereninsuffizienz, insbesondere infolge einer interstitiellen Nephritis, nicht unter allen Bedingungen zur Hochdruckbehandlung geeignet ist.
