Summary
In 186 patients with multiple myeloma the kidneys of 170 cases were examined morphologically. In 87 cases exact data on renal function were available revealing a normal excretory function of kidneys during the last 14 days in only 12.6% of all patients, 25.6% showed a slightly, 62.0% a severely reduced renal function. Uremia was the main cause of death in 32% of the deceased. When correlated with morphological findings pathological-anatomical evidence of myeloma kidney was noted in 44.8% of all patients with renal insufficiency, 32.8% however simultaneously showed the signs of acute renal failure. In about a third (28.9%) of all insuffizient kidneys, only signs of acute renal failure were evident, in 3.9% each a pyelonephritis or a contracted kidney could be implicated as the cause of renal dysfunction. When discussing the correlation between structure and function the unusually frequent picture of acute renal failure or tubular insufficiency must be taken into account when considering therapy. Due to careful control of excretory functions, appropriate prophylaxis and timely therapy uremia seemed to be a less frequent cause of death during the last two years.
Zusammenfassung
Von insgesamt 186 Patienten mit multiplem Myelom wurden 170 Nieren pathologisch-anatomisch untersucht. In 87 Fällen lagen genauere klinische Angaben zur Nierenfunktion vor, aus denen hervorgeht, daß nur 12,6% der Erkrankten in den letzten 14 Tagen des Lebens eine normale Ausscheidungsfunktion der Nieren, 25,6% eine leichte, 62% eine schwere renale Insuffizienz hatten. Bei 32% der Verstorbenen stand die Urämie als Todesursache im Vordergrund. Ein Vergleich morphologischer Befunde mit klinischen Daten zeigt, daß bei renaler Insuffizienz zwar in 44,8% der Fälle das pathologisch-anatomische Bild einer Plasmocytomniere vorhanden ist, in 32,8% jedoch gleichzeitig Anhaltspunkte für ein akutes Nierenversagen (a.N.) bestehen. In nahezu einem Drittel (28,9%) aller niereninsuffizienten Plasmocytomkranken sind allein die Zeichen eines a.N. zu erkennen und in jeweils 3,9% können eine schwere Pyelonephritis oder eine Schrumpfniere als Ursache der Nierenfunktionsstörung angesehen werden. In der Diskussion der Beziehungen von Struktur und Funktion wird im Hinblick auf therapeutische Konsequenzen auf das ungewöhnlich häufige morphologische Bild eines a.N. bzw. einer tubulären Insuffizienz hingewiesen und gezeigt, daß in den letzten zwei Jahren offenbar infolge sorgfältigerer Überwachungen der Ausscheidungsfunktionen, geeigneter Prophylaxe und rechtzeitiger Behandlung die Urämie seltener Todesursache beim Plasmocytom zu werden scheint.
