Primäre und sekundäre Laparotomie mit Splenektomie bei Patienten mit Morbus Hodgkin

Primary and secondary laparotomy with splenectomy in patients with Hodgkin's disease. preliminary results of the freiburg series

Summary

Between October 1969 and July 1972 primary staging laparotomies with splenectomy were carried out in 29 patients with Hodgkin's disease. In 41 cases secondary laparotomy with splenectomy was performed 1–12 years after the initial treatment. At primary laparotomy 16 of 29 cases showed involvement of abdominal lymph nodes and/or the spleen, but no hepatic involvement was found. At secondary laparotomy 4 of 27 cases (=15%) in clinical stages I and II (Ann Arbor) and 8 of 14 cases (=57%) in stage III (Ann Arbor) showed involvement of abdominal lymph notes and/or the spleen. Two cases with hepatic involvement were also observed. Secondary laparotomy with splenectomy is an appropriate method when initial treatment has not been completely successful and further therapeutic measures are necessary to correct this.

Zusammenfassung

Von Oktober 1969 bis Juli 1972 wurden bei Patienten mit Morbus Hodgkin in 29 Fällen eine primäre Staging-Laparotomie mit Splenektomie und in 41 Fällen eine sekundäre Laparotomie mit Splenektomie 1–12 Jahre nach Erstbehandlung durchgeführt. Bei der primären Laparotomie fand sich in 16 von 29 Fällen ein Befall von abdominellen Lymphknoten und/oder der Milz, dagegen kein Leberbefall.

Bei der sekundären Laparotomie fand sich im klinischen Stadium I und II (Ann Arbor) in 4 von 27 Fällen (=15%) und im Stadium III (Ann Arbor) in 8 von 14 Fällen (=57%) ein Befall von abdominellen Lymphknoten und/oder der Milz einschließlich von 2 Fällen mit Leberbefall.

Die sekundäre Laparotomie mit Splenektomie ist eine geeignete Maßnahme, unzureichende Behandlungen der zurückliegenden Behandlungsperiode nachträglich auszugleichen.
