Pediatric challenge case

Abstract

This 5-year-old boy presented to the emergency department with a 3-week history of upper abdominal pain. The child was afebrile, but he had been vomiting for 2 days. Abdominal and chest radiographs were obtained (Fig. 1), and because of the findings, an ultrasound of the chest and upper abdomen was performed (Fig. 2). What is the most likely diagnosis?

    Congenital diaphragmatic hernia

    Posterior mediastinal tumor with pleural effusion

    Right middle and lower lobe atelectasis
