Low-leg fractures in rabbits treated by cast only had consolidated within 20 to 22 days. Incipient calcification of callus was first seen on the 16th to 18th day following fractore.
The influence of calcergy, calciphylaxis, pyrophosphatase, and alcaline phosphatases to accelerate the mineralization of the callus was studied on the 12th day post fracture in 12 to 14 rabbits respectively. The calcergen, the challenger or the enzymes were injected into the fracture on the 8th day following fracture when collagen tissue formation in the fracture gap was present.
As seen by physical and x-ray examination on the 12th day best results were obtained by calciphylaxis and alcaline phosphatases. The poor results of pyrophosphatase are explained by the use of an acid preparation of pH 4.9. Microscopic examination of the callus revealed increased bone formation in the groups treated by calcergy, calciphylaxis, and alcaline phosphatases.
The experiments demonstrate the feasibility of influencing mineralization during the final phase of callus formation and suggest the possibility of shortening fracture healing. Injections of alcaline phosphatases into the fracture gap seem to promise best results at lowest risk among the methods employed.
Zusammenfassung
Unterschenkelbrüche bei Kaninchen heilten im Gipsverband nach 20 bis 22 Tagen fest aus. Beginnende Verkalkung des Callus fand sich im Röntgenbuld am 16. bis 18. Tag post fracturam.
An jeweils 12 bis 14 Kaninchen wurde die Wirkung der Calcergie, Calciphylaxie, Pyrophosphatase und alkalische Phosphatasen am 12. Tag nach der Fraktur auf die Beschleuningung der Callusmineralisation hin untersucht. Die intrafrakturären die Beschleunigung der Callusmineralisation hin untersucht. Die intrafrakturären Injektionen des Calcergens, des auslösenden Stoffes (challenger) oder der Enzyme erfolgten am 8. Tag post fracturam — also erst nach vorausgehender bindegewebiger Callusbildung.
Physikalisch und röntgenologisch ergaben die Calciphylaxie und die alkalischen Phosphatasen die besten Resultate. Das enttäuschende Abschneiden der Pyrophosphatase ist durch die Verwendung eines sauren Präparates (pH 4,9) zu erklären. Die histologischen Untersuchungen zeigten eine vermehrte knöcherne Callusbildung bei der Calcergie- und Calciphylaxiegruppe sowie den beiden mit alkalischen Phosphatasen behandelten Gruppen.
Die Versuche ergaben Beeinflussungsmöglichkeiten der letzten Phase der Frakturcallusbildung, der Mineralisation des bindegewebigen Callus. Damit eröffnen sich Aussichten auf eine Verkürzung der Frakturheilungszeit. Nach den bisherigen Versuchen scheint die intrafrakturäre Injektion von alkalischer Phosphatase den besten Erfolg bei geringster Gefährung zu versprechen.
