Thoraxfibrome

Zusammenfassung

An Hand von 11 Thoraxfibromen unserer Klinik wird auf die Problematik der benignen fibrösen intrathorakalen Tumoren eingegangen. Ihre Symptomatologie deckt sich im wesentlichen mit jener anderer benigner intrathorakaler Geschwülste. Lediglich endobronchiale Fibrome lassen sich präoperativ mit Sicherheit (durch Probeexcision) diagnostizieren. Die Therapie ist eine chirurgische und wegen der Möglichkeit maligner Entartung und Beeinträchtigung vitaler Funktionen durch Verdrängung von Mediastinalorganen absolut indiziert. Die Prognose nach radikaler Exstirpation des Tumors ist gut, Rezidive sind jedoch beschrieben. Bei den 5 von unseren 8 Fällen, teilweise 8 Jahre nach der Operation, durchgeführten Nachuntersuchungen konnten keine Rezidive gefunden werden. Die von amerikanischer Seite empfohlene Abgrenzung der Pleurafibrome (umschriebene fibröse Mesotheliome) von den übrigen intrathorakalen Fibromen können wir uns mangels spezifischer Gewebsstrukturen bei den von uns beobachteten 8 Fällen nicht anschließen. In dem uns zugänglichen Schrifttum konnten wir insgesamt—einschließlich unserer eigenen Fälle—166 intrathorakale Fibrome, wovon 68 der visceralen, 24 der parietalen Pleura, 50 dem Mediastinum, 14 der Thoraxwand und 10 der Lunge entstammten, zusammenstellen.
