Die Nierenfunktion bei der Endangiitis obliterans

Ein Beitrag zur Frage der Generalisation der Endangiitis obliterans und des endangiitischen Hochdruckes

Zusammenfassung

  1. 1.

    Die Endangiitis obliterans (v.Winiwarter-Buerger) stellt eine Allgemeinerkrankung des ganzen Gefäßsystems, gelegentlich unter besonderer Bevorzugung einer oder mehrerer Gefäßprovinzen dar. Die Stellung der Nieren im Rahmen des umfassenden Gefäßleidens wird erörtert.

    Im Verlauf einer Endangiitis obliterans kann, bedingt durch endangiitische Gefäßveränderungen der Aorta, der A. renalis oder intrarenaler Gefäße, ein Hochdruck auftreten und zwar auch in kurzer Zeit. Auf den einer chirurgischen Therapie zugänglichen endangiitischen Hochdruck, dessen Symptomatik und Diagnostik wird hingewiesen.

    Bei 51 Patienten mit peripheren Durchblutungsstörungen—35 Endangiitikern und 16 Patienten mit peripheren Durchblutungs-störungen anderer Genese zum differentialdiagnostischen Vergleich—wurden funktionelle Nierenuntersuchungen mit Ausscheidungs-und Clearance-Methoden durchgeführt. Die endogene Kreatinin-Clearance ließ bei der Endangiitis obliterans ein kennzeichnendes Verhalten nicht erkennen. Der effektive Nieren-Plasmafluß und die gesamte Blutdurchströmung der Nieren waren in allen Altersgruppen stark eingeschränkt. Die Phenolsulfonphthaein-Ausscheidung war als sekundäre Auswirkung einer primären Minderdurchblutung der Nieren herabgesetzt.

    Die funktionellen Nierenstörungen gehen der endangiitischen Organmanifestation voraus.
