Über Neurinome des Magens

Zusammenfassung

Geschwulstbildungen des Magens sind überaus häufig. Im Gegensatz zu den vorwiegend malignen Geschwülsten nehmen hierunter die benignen Tumoren nur einen kleinen Teil ein, der im eigenen Krankengut mit 1600 Carcinomen und Sarkomen nur knapp 3% ausmacht. Unter 48 benignen Tumoren fanden sich neun gutartige neurogene Geschwülste. Außerdem wurde ein malignes Neurinom beobachtet.

Die klinische Erkennung der Magenneurinome ist selten möglich. Sie verursachen meist erst beim Auftreten von Komplikationen Erscheinungen, die vorzugsweise in Blutungen bestehen. Die Diagnose läßt sich makroskopisch häufig auch bei der in jedem Fall indizierten Operation nicht klären. Eine Unterscheidung von malignem Tumor oder gutartigem Neurinom ist vielmehr nur histologisch unter Zuhilfenahme spezieller Färbemethoden möglich. Die intraoperative Untersuchung im Schnellschnittverfahren hat sich für die Wahl des operativen Vorgehens bewährt.

Die Prognose der Erkrankung ist günstig, sofern die radikale Entfernung des tumortragenden Magenabschnittes einer, wenn auch seltenen, malignen Entartung zuvorkommt. Anhand von zehn beobachteten Fällen werden Symptome, Klinik, Behandlung und postoperative Ergebnisse geschildert.
