Probleme bei der Behandlung von Galleabflußstörungen im distalen Gangapparat und an der Papille

Zusammenfassung

Zweifellos hat die Behandlung von Galleabflußstörungen im distalen Gangapparat und an der Papille durch Bereicherung der Diagnostik neue Impulse erfahren. Aus der Ermittlung eines anatomischen Hindernisses an der Papille ergibt sich die Konsequenz für unser operatives Handeln. Der Weg bei juxta-papillären Hindernissen und beim Papillentumor ist technisch vervollkommnet, hat sich aber im Grunde seit Jahrzehnten nicht geändert. Eine lebhafte Diskussion haben die Maßnahmen am Sphincterapparat als Operationsmethodik bei Abfluß-störungen uns dem Gallengang ausgelöst. Der direkte Eingriff an der Papille ist unter mehreren Möglichkeiten ein anatomisch wohlbegründeter Weg, der auch unseren funktionellen Vorstellungen Rechnung trägt. Über Fragen der Indikation sind die Akten noch nicht geschlossen. Grundsätzlich den Wert der Eingriffe am Sphincter leugnen zu wollen, müßten wir als Rückschritt empfinden.
