Zusammenfassung
Herzfehler mit anscheinender Verdoppelung des linken Vorhofs und Abgrenzung der entstandenen Gebilde durch ein sog. anomales Septum (Cor triatriatum) wurden früher selten beschrieben. Sie wurden aber in den letzten Jahren häufiger beobachtet und haben durch die Entwicklung der Herzchirurgie praktische Bedeutung erlangt.
Der überzählige Vorhof kann 1. durch einen stark erweiterten distal stenosierten Lungenvenenstamm, 2. durch einen infolge totaler Lungenvenefehlmündung kammerartig vergrößerten Sinus coronarius und 3. durch einen persistierenden linken Ductus Cuvieri mit starker Erweiterung des Sinus coronarius vorgetäuscht werden.
Es wird versucht, eine Einteilung auf Grund der verschiedenen Hämodynamik zu geben.
Morphogenese, klinische Symptome und Therapiemöglichkeiten werden besprochen. Mit Ausnahme des persistierenden Ductus Cuvieri stellen die anderen Formen des C. t. eine absolute Operationsindikation wegen der sonst sehr schlechten Prognose dan.
In drei eigenen Fällen wurde der überzählige linke Vorhof durch einen stark erweiterten Sinus coronarius vorgetäuscht, davon 2mal in Verbindung mit einer totalen Lungenvenenfehlmündung, im 3. Fall bei persistierendem linken Ductus Cuvieri.
