Summary
It is reported on the 87 children with a congenital atresia of the oesophagus who came for treatment between 1952 to 1966. An operation was performed in 78 children. The average lethality amounts to 50%. It shows a decrease from year to year and was 27,3% in the last years.
According toWaterston the children can be divided in three groups with the three following factors as the important aspects of the different groups: Maturity, pneumonia and additional malformations. The group A is composed of mature children without a pneumonia or other malformations, group B children with a birthweight up to 2500 g with an average pneumonia and slight malformations, and group C children with a birthweight under 1800 g, with serious pneumonia and severe malformations. The lethality in group A is 0 to 6% and in group C between 80 to 95%. Our figures correspond to those given in the literature. The above mentioned factors which influence the results are discussed and references to improvement are also mentioned e.g. the staged-procedure by the American paediatric surgeons.
Zusammenfassung
Es wird über 87 Kinder mit einer kongenitalen Oesophagusatresie berichtet, die in den Jahren 1952 bis 1966 zur stationären Aufnahme kamen. 78 Kinder wurden operiert. Die durchschnittliche Letalität beträgt 50%. Eine Jahreseinteilung läßt erkennen, daß die Sterblichkeit kontinuierlich abfällt. In den letzten Jahren betrug sie 27,3%.
Unter Berücksichtigung dreier wesentlicher Faktoren—Reifezustand, Pneumonie und zusätzliche Mißbildungen—wird nach dem VorschlagWaterstons eine Gruppeneinteilung getroffen, wonach ausgetragene Kinder ohne Pneumonie und ohne zusätzliche Mißbildungen in Gruppe A, Kinder mit einem Geburtsgewicht bis 2500 g, mit mittelschwerer Pneumonie und leichten Mißbildungen in Gruppe B und Kinder unter 1800 g mit schwerer Pneumonie und schweren Begleitmißbildungen in Gruppe C kommen. Es ergibt sich, daß die Sterblichkeit in Gruppe A bei 0 bis 6% liegt, in Gruppe C zwischen 80 und 95%. Die eigenen Zahlenangaben stimmen mit denen der Literatur weitgehend überein. Die genannten Faktoren, die die Ergebnisse wesentlich beeinflussen, werden der Reihe nach besprochen und Ansätze zur Verbesserung angedeutet, wie z. B. das mehrzeitige Operationsverfahren amerikanischer Kinderchirurgen.
